About this file

The document is a Living Wage Certification Form from the Town of Manchester, Connecticut, related to a contract for furnishing and installing a sodium hypochlorite storage tank at the Manchester Wastewater Treatment facility. The form requires bidders to certify whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212 of the Manchester Code of Ordinances) and commit to paying the required living wage to eligible employees during the contract term.

The certification form provides three potential exemption categories for bidders: (1) charitable foundations or nonprofits exempt from federal income taxation, (2) bidders employing fewer than 25 eligible employees, or (3) contracts with an annual value less than $25,000. Bidders must indicate their status by checking the appropriate box, with a clear warning that failure to indicate their status may result in bid rejection. The form must be signed by an officer, owner, or authorized representative and returned with the bid or RFP submission, serving as a formal attestation of the company's compliance with local wage standards.
