BASIS FOR AWARD: The government will make award to the Lowest Priced Technically Acceptable quote with relevant satisfactory past performance. Quotations will be evaluated on the following factors:

Technical, Past Performance, Delivery and Price. A submitted quote must conform to all specifications and delivery requirements to be considered technically acceptable. Quoters are cautioned to ensure that submitted quotes contain their best terms from both a technical and pricing standpoint. Evaluation factors will be evaluated on an acceptable/unacceptable basis.

TECHNICAL COMPLIANCE: To ensure technical compliance:

a. A mere statement that the quoter will meet the Government’s requirements is NOT sufficient.

b. Quoters must clearly state ability to deliver the estimated quantities of LP gas to the identified locations.

c. The Government requires delivery of LP Gas as necessary to keep tanks at adequate operating levels.

d. Quoters must clearly state ability to meet the required delivery time. Vendors that cannot meet the required delivery time WILL NOT be considered technically acceptable.

PAST PERFORMANCE: The quoter shall provide at least three (3) relevant past performance references. Relevant past performance is defined as performance of contracts of similar size, scope, and complexity within the past three (3) years. For the purpose of this RFQ, “similar in scope” means the provision, delivery, and installation of a similar quantity. Reference information shall include the following for which your firm has provided same/similar services or supplies.

Agency name

Contract number(s)

Dollar value

Brief description of the work completed

Point of contact information (to include email address)

Past performance information will be obtained from the quoter’s references. Make sure your reference information is up to date. DO NOT GIVE THE NAME OF REFERENCES UNLESS YOU HAVE MADE

SURE THAT THE INDIVIDUAL CAN BE REACHED AT THE REFERENECED TELEPHONE NUMBER,

E-MAIL ADDRESS, OR POSTAL ADDRESS THAT YOU HAVE PROVIDED.

The Government reserves the right to obtain past performance information from sources other than those provided and to use the information in the evaluation of quoters. The Government will evaluate the quoter’s, and (if applicable) its principle subcontractor’s demonstrated past performance in delivering quality services and in meeting schedule requirements on similar programs. The performance risk assessment will address the probability of the quoter successfully accomplishing the contract requirements based on a relevant past performance. The currency and relevance of the information, source of the information, context of the data, and general trends in contractor’s performance will be considered. Problems not addressed by the quoter will be considered to still exist. The degree to which the quoter can demonstrate that it has successfully applied systematic improvement to resolve past performance problems will be evaluated.

Each quoter will be evaluated to the extent to which they demonstrate satisfactory or better performance on contracts or subcontracts currently ongoing or completed within the last three (3) years. References other than those identified by the quoter may be contacted by the Government and used in evaluating the quoter’s past performance. Past performance is an indicator of how current and/or previous customers asses contractor performance under a specific contract performed for them and address the full range of all functions required to make any contractual relationship work. Successful past performance indicates that a contractor has the resources and skills necessary to perform the specific work requirements of this solicitation.

Evaluation of past performance will be based on consideration of all relevant facts and circumstances.

The evaluation will include demonstrated past performance in quality of product or service, schedule, and customer satisfaction.

In the case of a quoter without a record of relevant past performance or for whom information on past performance is not available, the quoter may not be evaluated favorably or unfavorably on past performance. Quoters without a record of relevant past performance will receive a neutral rating.

FAR 52.217-5 Evaluation of Options (Jul 1990)

The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. Evaluation of options will not obligate the Government to exercise the option(s).

“Commercial and Government Entity (CAGE) code” means—

(1) A code assigned by the Defense Logistics Agency (DLA) Logistics Information Service to identify a commercial or Government entity; or

“Data Universal Numbering System (DUNS) number” means the 9-digit number assigned by Dun and Bradstreet, Inc. (D&B) to identify unique business entities.

“Data Universal Numbering System+4 (DUNS+4) number” means the DUNS number means the number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this 4-character suffix.) This 4-character suffix may be assigned at the discretion of the business concern to establish additional SAM records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at

Subpart 32.11) for the same concern.

“Registered in the SAM database” means that—

(1) The Contractor has entered all mandatory information, including the DUNS number or the DUNS+4 number, into the SAM database;

(2) The Contractor’s CAGE code is in the SAM database; and

(3) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification

CAGE code and DUNS or DUNS+4.

(3) If indicated by the Government during performance, registration in an alternate system may be required in lieu of

SAM.

(c) If the Contractor does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one.

(1) A contractor may obtain a DUNS number—

(f) The Contractor is responsible for the accuracy and completeness of the data within the SAM database, and for any liability resulting from the Government’s reliance on inaccurate or incomplete data. To remain registered in the

SAM database after the initial registration, the Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the SAM database to ensure it is current, accurate and complete. Updating information in the SAM does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document.

(g)(1)(i) If a Contractor has legally changed its business name, “doing business as” name, or division name

(whichever is shown on the contract), or has transferred the assets used in performing the contract, but has not completed the necessary requirements regarding novation and change-of-name agreements in Subpart 42.12, the
