PERFORMANCE WORK STATEMENT
MARFORPAC Information Environment Division
1.0 SCOPE: The Contractor shall provide the technical expertise and skills to provide the preventive maintenance, scheduling through MARFORPAC’s government appointed representative and respond to
Emergency Maintenance requests resulting in inoperative or failed components or loss in voice communications. Support required for base year with 3-4 option years. U.S. Marine Corps Forces Pacific
(MARFORPAC) Headquarters has a requirement for certified AVAYA Federal Business Partner to routine preventive maintenance and emergency maintenance to the Private Branch Exchange (PBX) telephone switch which supports over 750 MARFORPAC Staff members with voice communications. It is imperative that the MFP has 24/7 phone system operation to ensure the mission of the Organization, Command and Control.).
2.0 CONTRACT OVERVIEW: Work shall be limited to the Provide maintenance services, including emergency, preventative and routine service, to the Avaya CM6 in Bldg 81 and components to include
(4) Expansion Port Networks (EPNs), in Bldg 1, 80, 600, and Avaya Modular Messaging System and located at MFP IE DIV TeleComm Building 81 room 1, 2nd floor, Camp smith Hi 96861. Avaya emergency support request will be responded to with in 3 hours, and regular maintenance support and service will be scheduled with the Technical POC quarterly and Service Work Order documentation provided for every service call whether scheduled or emergent in nature. Description of work will be annotated in work order and provided to Government POC.
3.0 PERFORMANCE REQUIREMENTS:
3.1 Task 1: Full Maintenance 8x5 Coverage
Contractor shall be available during Standard Business Hours, 8 a.m. to 5 p.m., in the time zone of the covered products, Monday through Friday, excluding Avaya and Government holidays. Coverage shall include remote telephone support, remote diagnostics, troubleshooting, problem resolution, software maintenance updates/fixes, on-site parts replacement (if the covered product includes hardware), and any on-site support Avaya deems necessary to resolve a fault. Requests for support outside the Standard
Business Hours may be accommodated at Avaya’s option and the applicable Per Incident Maintenance rate. The response objective for failures which materially affect the operation for failures which materially affect the opration
3.2 Task 2: Maintenance Per Incident Support (MPI)
Avaya provides ordering activities with Maintennance Per Incident Support on a per-call basis if the ordering activity has a maintenance agreement or when ordering activities require assistance outside the coverage terms of their service order or warranty. Maintenance Per Incident Support Services are available for help line, administration, diagnostics, and on-site service support. Support is extended to all products supported by Avaya. Maintenance Per Incident Support offers and associated charges are specified in the price list.
4.0 CONTRACTOR QUALIFICATIONS: The Contractor shall be an Avaya Certified
Telecommunication Technician.
5.0 WORK HOURS/PERSONNEL, SHIFT SCHEDULES:
5.1 Normal business hours of operation for MARFORPAC IE Div are Monday-Friday, 0630 to 1630 HST, excluding Government holidays. Contractor website access shall be available 24 hours a day, seven days per week.
5.2 Holidays. A portion of the effort under this contract may be performed on a government installation. For purposes of scheduling personnel, the Contractor is hereby advised that the government installation will observe the following holidays. The Contractor is further advised that access to the government installation may be restricted on these holidays.

5.3 In addition to the days designated as holidays, the Government observes the following days:
5.3.1 Any other day designated by Federal Statute
5.3.2 Any other day designated by Executive Order
5.3.3 Any other day designated by a Presidential Proclamation
5.3.4 Any other period of time as designated by appropriate installation authority.
6.0 PLACE - PERIOD OF PERFORMANCE: Except for meeting attendance at MAFORPAC IE Div, Camp Smith, HI, the work shall be performed at the Contractor’s site.
Base Year: 1 year from award date
Plus (3) 1 year option periods
7.0 SECURITY CLEARANCE/CONTRACT SECURITY REQUIREMENTS
No classified work will be conducted at the contractor’s facility. Contractors may be required to participate in classified meetings and discussions up to the Secret level held only at U.S. Marine Forces, Pacific (MARFORPAC), Camp H.M. Smith, HI. Contractors participating in meetings and discussion at
MARFORPAC must be U.S. Citizens cleared to the Secret level.
7.1 The Contractor shall comply with all current MARFORPAC/Camp Smith base security requirements. At any time, to include heightened Force Protection Conditions, natural or manmade disasters, and/or any other security concern, the Commanding Officer of MARFORPAC/Camp Smith has the sole authority to change current security requirements. These changes could impact the original PWS, and the contractor’s immediate compliance will be required at no additional cost to the Government
7.2 Only citizens of the United States and Permanent Resident Aliens of the United States with a verified
United States Permanent Resident Card (USCIS Form I-551) are eligible to perform work on this contract at the contractor’s facility. All other foreign nationals are not eligible to perform work on this contract.
Foreign nationals from the following countries are automatically not permitted to be employed under this contract: Iran, Iraq, Syria, Sudan, Somalia, Libya, and Yemen. Contractors attending meetings at
MARFORPAC Camp H.M. Smith must be U.S. citizens with a Secret security clearance. Permanent
Resident Aliens of the U.S. may not participate in these meetings.
NOTE: Since all the work to be performed at the contractor’s facility is unclassified there is no need to restrict the work to only U.S. citizens, however, the meeting at MFP may be at the classified level so only
U.S. citizens with Secret access can attend these meetings so hopefully this makes sense.
7.3 Contractor shall comply with all applicable DoD security regulations and procedures during the performance of this contract. Contractor shall not disclose and shall safeguard classified information, procurement sensitive information, computer systems and data, Privacy Act data, controlled unclassified information, and all government personnel work products that are obtained or generated in the performance of this contract. The Contractor shall take all lawful steps available to ensure that information provided or generated pursuant to this arrangement is protected from further disclosure unless the agency provides written consent to such disclosure.
7.4 All Contractor personnel performing work under this contract who require access to MARFORPAC/Camp
Smith shall obtain personal entry and vehicle passes from the Provost Marshall Office, Camp Smith. The
Commanding Officer has broad authority to remove or exclude any person from the military installation to protect personnel and property, to maintain good order and discipline, and to ensure the successful and uninterrupted performance of the Marine Corps mission. In the exercise of this authority, the
Commanding Officer may refuse to grant personal entry passes or may bar Contractor employees, including employees who have been granted a personal entry pass. Refusal to grant an employee a personal entry pass or debarment of an employee does not relieve the Contractor of the responsibility to continue performance under this contract.
7.5 Contractor personnel using personally operated vehicles on DoD installations shall have proper permits for entry onto the installations. All vehicles, private or contractor-owned, shall comply with the vehicle operation regulations that govern installations. All vehicles are subject to search while on DoD installations. Only licensed Contractor personnel shall operate vehicles on DoD installations. Vehicles shall be operated in accordance with local and state laws as well as installation-specific traffic regulations.
7.6 The Contractor’s employees shall observe and comply with all base rules and regulations applicable to contract personnel, including those applicable to the safe operation of vehicles, and shall not be present in locations not required for the proper performance of this contract. Contractor personnel and equipment entering a military installation are subject to security checks and/or searches. Contractor personnel shall follow any direction given by Military Police or other security or safety personnel.
7.7 Removal Requested by the Contracting Officer (KO): The KO may require the Contractor to remove an employee working under this contract for reasons of misconduct or security violations. Contractor employees shall be subject to dismissal from the premises upon the determination by the KO that such action is necessary for the interest of the Government.
8.0 SAFETY
8.1. The Contractor is solely responsible for compliance of all safety regulations of employees while aboard government-owned facilities. All accidents which may arise out of, or in connection with, the performance of services required hereunder which result in injury, death, or property damage, shall be reported in writing to the KO within twenty-four hours of such occurrence. Reports shall provide full details of the accident, including statements from witnesses. The foregoing procedures shall also apply to any claim made by a third party against the Contractor as a result of any accident that occurs in connection with performance under this contract.
9.0 LANGUAGE REQUIREMENTS: All Contractor personnel shall be sufficiently competent in reading, writing, speaking, and understanding English to perform the work.
10.0 PERSONNEL CONDUCT: The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor personnel employed to perform tasks specified herein shall be the responsibility of the Contractor. The Contractor shall be responsible for the performance and conduct of Contractor and subcontractor employees at all times. Personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering the installation, shall abide by the security regulations applicable to the installation or listed in the contract and shall be subject to such checks by the
Government as deemed necessary. The Contractor shall not employ for performance under this contract any person whose employment would result in a conflict of interest with the Government’s standards of conduct.
11.0 NON-PERSONAL SERVICE STATEMENT: Contractor employees performing services under this order will be controlled, directed, and supervised at all times by management personnel of the Contractor.
Contractor management will ensure that employees properly comply with the performance work standards outlined in the performance work statement. Contractor employees will perform their duties independent of, and without the supervision of, any Government official or other defense-contractor. The tasks, duties, and responsibilities set forth in the order may not be interpreted or implemented in any manner that results in any Contractor employee creating or modifying federal policy, obligating the appropriated funds of the
United States Government, overseeing the work of federal employees, providing direct personal services to any federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal
Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work.
