Performance Work Statement for (PWS)
For
Lumenis Ultrapulse TotalFX Laser Maintenance Support Services
1. DESCRIPTION OF SERVICES.
1.1 Requirements. The contractor shall provide service for the Lumenis Ultrapulse
TotalFX Laser system. Service provided shall be software and maintenance support services for the Ultrapulse Laser at Wilford Hall Medical ASC, JBSA Lackland, Texas.
1.1.1. Specific Tasks. The contractor shall provide tasks to include, but are not limited to:
1.1.1.1. The contractor shall provide the following basic support services (collectively, “Services”) to Wilford Hall ASC for the Lumenis Ultrapulse Laser by telephone and remote support to resolve issue/problem. If the issue/problem cannot be repaired in a timely manner through telephone and remote support, then the contractor shall respond to service call within two business days of service call initiation. Downtime of system will be kept to an absolute minimum. The contractor must notify the customer of all projected downtime and estimated time of repair. Lumenis shall use reasonable efforts to provide on-site field service support within two business days ours of customer’s initial call. The contractor shall provide technical phone support services Mon – Fri 8:00-5:00.
1.1.1.2. Preventative Maintenance. The contractor shall provide preventative maintenance including a system verification of the unit. If the contractor determines that alignment, adjustment and/or replacement of parts are required, the contractor shall perform these services at no additional cost to the government.
1.1.1.3. Upgrades, modifications and software enhancements shall be made available to the government at the time of release, at no additional cost to the government.
1.1.1.4. Emergency Support. The contractor shall provide emergency on site and depot repairs within two business days of initial call.
1.1.1.5. The contractor shall provide software updates to operate the dermatology
Ultrapulse TotalFX laser system.
1.1.1.6. Parts and labor required will be provided at the time of service, at no additional cost to the government. All parts replaced will meet original equipment manufacturers’ specification.
1.1.1.7. Safety Modifications. The contractor shall provide all equipment safety modifications required throughout the term of the contract at no additional cost to the government.
2. General Information.
2.1. QUALITY ASSURANCE. The government will evaluate the contractor’s performance by appointing a representative to monitor performance monthly to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections and receipt of complaints from base personnel. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. The government will also receive and investigate complaints from various customers located on the installation, the contractor shall be responsible for initially validating customer complaints. However the government representative shall make final determination of the validity of customer complaints in cases of disagreement with customer.
2.2. GOVERNMENT REMEDIES. The contractor officer shall follow the requirements of FAR 52.212- 4, Contract Terms and Conditions for Commercial Items
(May 1997), for contractor’s failure to correct nonconforming services.
2.3. HOURS OF OPERATION. The contractor shall provide support services and technology 7 AM to 5 PM central time, Monday thru Friday.
2.3.1. The contractor shall respond to service call within 48 hours of service call. Down time of system will be kept to an absolute minimum. The contractor must notify the customer of all projected downtime and estimated time of repair.
2.4. SECURITY REQUIRMENTS. The Contractor will supply the COR with full name
(as is on drivers license), drivers license number, date of birth and date of service to be provided access onto MTF at time of service.
2.4.1. Contractor personnel shall park in the appropriately designated parking areas as determined by the COR. The Government will not invalidate or reimburse for Contractor parking violations under any condition.
2.4.2. Contractor personnel operating motor vehicles on Federal property shall possess an operator’s license for the category of vehicle being operated and automobile issuance as required by regulations/laws governing said Federal property.
2.5. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE
NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT
COMMANDER. In the event that National Command authority has declared a crisis, Contractor shall provide all services within this SOW, as stated under non-crisis conditions.
2.6. Qualifications. The contractor’s technician shall meet the following requirements:
2.6.1. Have been trained on the specific piece of equipment they service by the OEM.
All contracted Equipment Service Technicians shall be trained and certified to work on the equipment by the contractor.
2.6.2. The Contractor shall not employ current DOD employees, military or civilian, to provide services under this contract.
2.7. Compliance. The contract personnel shall comply with all applicable Air Force
Instructions (AFI), and Department of Defense (DOD) Regulations.
2.7.1. Government Data. The contractor shall manage all data created for Government use or legally controlled by the Government, in support of the functional activity or required by AF publications, IAW with the AF records management procedures in AFI
33-322, Records Management Program, paragraph 10; AFMAN 33-363, paragraph 6.4.;
AFI 33-364; paragraph 2.10. and the Air Force Records Disposition Schedule (AF RDS) located at: www.af.mil/shared/media/document/AFD-080429-051.pdf.
2.7.2. The Contractor will only accept calls for services by the COR. The phone numbers for contacting the CORs at Contract Services are:

3. HEALTH REQUIREMENTS AND IMMUNIZATIONS.
4. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
4.1. Facilities and support Services. The government will provide utilities for equipment operation.
4.2. Telephone. The Government will provide local telephone service, Class-A telephone lines and Defense Switching Network lines limited to matters related to the performance of this contract. Personal long distance calls are not authorized.
4.3. Badges. The government will provide MTF Visitor ID badge and will be worn/displayed at all times.
