Statement of Work (SOW) FOR
GROUNDS AND TREE MAINTENANCE SERVICES FOR
USCG TX SITE USN TRANSMIT FACILITY LLL EDISON ST. BLDG. 471 WAIANAE, HAWAII
1. DESCRIPTION OF SERVICES
The Contractor shall provide all labor (including management and supervision), material, equipment, tool, supplies, transportation, and personnel for grounds maintenance and tree trimming services for USCG located on the Naval Installation, TX SITE USN TRANSMIT FACILITY LUALUALEI, WAIANAE as cited on the attached map. Land area is approximately 54,425 SF, The ground, shall be properly maintained and present a clean, neat, and professional appearance.
MAINTAIN GROUNDS
1.1 GRASS CUTTING
Contractor shalt cut/mow the grass within the areas cited oh the attached map. Grass trimmings shall be removed or mulched when visible after mowing, Contractor shall maintain the growth of grass height depending upon the type of grass. The following height is a guideline for a neat and professional appearance. All grounds shall look well-manicured at all times.
St. Augustine/Bermuda - 2 to 3 inches in height.
1.2 EDGING
Sidewalks, driveways, curbs, and other concrete or asphalt edges located on the facility shall be edged concurrent with mowing. Edging shall include removal of vegetation from cracks in sidewalks, driveways, _and curbs within 0.5 inch of the edged surface and to a depth of 2.0 inches.
1.3 TRIMMING
Grass shall be trimmed around trees, shrubs, buildings, fences, poles, posts, fire hydrants, parking lot bumper blocks, boulders, and other fixed obstacles. Trimming height should match surrounding area grass heights. All areas shall be trimmed concurrent with mowing, Damage to trees and shrubs from trimming shall be repaired by the contractor. Contractor shall trim all trees and shrubs within the area that may pose as a hazard to the building/structure. If a plant should die or become unhealthy due to damage, the Contractor will be responsible for replacing the damaged plant with a plant of the same type and size.
1.4 DAMAGE TO PROPERTY
Areas damaged by the Contractor vehicles shall be seeded, sprigged or sodded to meet the standards of surrounding areas. Contractor shall also be responsible for damage of underground sprinkler pipes and sprinkler heads including Coast Guard property and/or personal property of Coast Guard members.
1.5 REPAIRS AND/OR REPLACEMENTS
All operations performed by the Contractor shall be conducted in a manner that assure the trees, shrubs, ground cover lawns, or any Government property existing in the area shall not be damaged. If damage should occur the Contractor shall replace the same as directed by the Contracting Officer or COR. The Contractor shall reimburse the Government for the replacement of the same at the direction of the Government. If damage occurs to the sprinklers system and or to the water distribution system while performing the operation under the contract the Contractor shall notify the COR or Base Honolulu Engineering Department immediately at 608-842-2903 or 808-291-845 7, the Contractor shall then make the necessary repairs to include but not limited to the sprinklers or water distribution systems at no additional expense to the Government.
1.6 SCHEDULING
Ten (10) days prior to contract start date and seven (7) days before each month begins thereafter, the Contractor shall submit a monthly schedule to the Contracting Officer's Technical Representative (COR) listing all the work days be performed the following month, This schedule shall allow the Government opportunity to make proper inspections before, during, and upon completion of each activity. Any change to the schedule will require written request by the Contractor and submitted to the COR, a minimum of forty-eight (48) hours of notice before a change in schedule. All work shall be performed within the normal five-day work week (Monday through Friday). No work shall be performed on weekends unless prior approval from the COR or Contracting Officer has been obtained. These scheduling guidelines apply to ALL services provided by the Contractor. Failure to provide and follow schedules shall be presumed to be a failure to perform specified work, since it will be impossible to check every area and task for performance every day.
1.6 FREQUENCY OF WORK
The work described herein must be performed approximately every 1st Wednesday and 3rd Wednesday of each month, or as needed to perform by contractor to meet standards of the PWS statement.
2. PERSONNEL
2.1 WORK FORCE
The Contractor shall provide an adequate work force with sufficient knowledge and experience to perform all services prescribed by the terms of the contract.
2.2 VEHICLES
Vehicles of the Contractor and the Contractor's employees shall be registered at the U.S. Naval installation. Contractor vehicles shall have signage on vehicles for identification. A current driver's license and no fault insurance will be required for all personnel who operate vehicular equipment within the boundaries of U.S Navy TRANSMIT FACILITY LLL WAIANAE, HAWAII.
2.3 EMPLOYEES
All personnel employed by the Contractor shall be properly trained and must be able to communicate sufficiently in the type of work involved and shall work under the direction of the Site Foreman. No later than five (5) days after contract award, the Contractor shall submit to the Contracting Ot1icer, and to the COR, a roster of his employees, which shall include the employee's full name, home address, and emergency telephone number, The Contracting Officer reserves the right to require the Contractor to _ remove from-·this contract any employee the Government deems to be incompetent, careless, insubordinate, or otherwise objectionable.
2.4 ATTIRE
The Contractor's employee's attire shall be as neat and clean as practical while performing their duties. The Contractor shall require all employees to wear distinctive uniforms clothing for ready identification. · .Employees shall l wear uniforms consisting of shirts and trousers/slacks or coveralls for men and women. The uniform shall have the Contractor's name in sufficient size to be read at a distance of approximate ten (10) feet, affixed thereon in a permanent manner. Employees shall also wear their government issued identification card attached to their uniform in such a way that their picture and name are visible.
2.5 LOITERING
Contractor's employees shall not use recreational equipment or loiter in or around buildings.
2.7 EMERGENCY REPORTING
Contractor or Contractor's employees shall report fires, hazardous conditions, forced entry and or vandalism to Naval Base Security Lualualei (668-7114) and Base Honolulu Officer of the Day (00D) at 808-226-4170, and COR at 808-842-2903/808°29 l-8457.
2.8 SITE FOREMAN
No later than five (5) days after the contract award; the Contractor shall submit, in writing, to the Contracting Officer and to the COR, the name of the Site Foreman. The Site Foreman shall conduct overall management coordination and shall be the central point of contact with the Government for performance of all work under the contract. The Site Foreman must be authorized to receive any notice of deficiency in contract performance. These notices may include, but are not limited to, the following: non-performance of work, deficiencies in work performed, and notices of propose payment deductions for and deficiencies.
2.9 ASSISTANT SITE FOREMAN
The Contractor may submit, in writing, to the Contracting Officer and to the COR, the name(s) of Assistant Site Foreman. All Assistant Site Foreman must be fully authorized to perform all of the above functions of the Site Foreman in his absence, The Site Foreman or alternate shall be available during normal duty hours to meet on the installation with the COR to discuss problem areas as needed.
2.10 SERVICE DELIVERY SUMMARY
The Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.
3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
The Government will furnish the Contractor reasonable amounts of electrical utilities in furtherance of the performance of work. Failure of the Government to furnish the utilities at any time during the performance of the contract due to outages· or other interruptions shall not be used by the Contractor as a basis for a claim against the Government. In the event of a power outage or interruption, the Contractor is not relieved from performing the tasks that do not require the utility. Contractor operations shall minimize the use of utilities. The Government will furnish no other property or service.
3.1 CONTRACTOR FURNISHED ITEMS OR SERVICES, The Contractor shall furnish all supplies, items, and equipment necessary for the performance of the contract, except for those items described in Paragraph 3, ·
3.2 LIABILITY FOR LOSS OR DAMAGE.
The Government shall not be liable for any loss, breakage, or damage to Contractor's equipment or supplies that may result by being stored by the Contractor on the Government facility.
3.3 SAFETY
All grounds maintenance equipment shall be maintained and used in a safe and effective manner, including effective safety shielding and protective equipment. The Contractor shall conform to all manufacturers safety precautions. Operator eye and hearing protection shall be provided by the Contractor and worn at all times during operations of mechanical lawn edger's, trimmers, and other power equipment, or where deemed necessary by Base Honolulu Safety Officer. Additionally, the Contractor shall be responsible for instructing all employees on OSHA safety measures, and report all accidents to the COR; in which there is any damage to personnel or property, within a period of twenty-four (24) hours of the damage.
Contractor's employee's shall observe and adhere to all safety or warning signs posted throughout the Government facilities.
3.4. SUPPLIES AND MATERIALS
The Contractor shall furnish all maintenance supplies and materials that must meet all Federal, State, and local environmental and safety requirements,
4. GENERAL INFORMATION
Contractor shall comply with all Federal, State and Local laws, regulations, ordinances, statues and regulations pe1taining to services provided,
4.1. QUALITY CONTROL
The Contractor shall develop and maintain a Quality Control Program to ensure maintenance services are performed in accordance with commonly accepted commercial practices. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-reoccurrence of defective services.
4.2 QUALITY ASSURANCE
All findings of unsatisfactory or non-performed work will be administered under the terms and conditions of this contract. Any re-work shall be corrected within a reasonable time of verbal notification; all costs associated with re-work are the responsibility of the Contractor.
4.3 HOURS OF OPERATION.
The Contractor's work shall be performed Monday through Friday between the hours of 8:00 a.m. and 4:00 p.m. (0800-1600 hours).
4.4 RECOGNIZED FEDERAL HOLIDAYS
The Contractor is not required to provide service on the following days: New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving and Christmas. If scheduled work falls on a holiday, services shall be completed on the next workday.
4.5 USE OF GOVERNMENT PREMISES.
The Contractor will not be provided storage for any equipment or supplies and non-hazardous materials at the site. Hazardous materials belonging to the Contractor will not be stored on property. Any hazardous materials brought on U.S Naval installation must be stored in an approved safety container (oil, gas, etc) and be of sufficient quantity to perform the daily tasks.
4.6 PERMITS AND RESPONSIBILITIES FOR WORK.
The Contractor shall, without additional expense to the Government, obtain all licenses and permits required for the performance of the work. Contractor shall be responsible for all damages to persons and/or property that occur as a result of their fault or negligence in connection with the performance of the work.
4.7 SITE VISIT
The Contractor shall visit the site of the work to confirm their understanding of the nature and location of the work; the general and local conditions; accessibility to locations where work must be performed; location of water/irrigation lines, sprinkler heads, electrical conduit, installed equipment and other items affecting the performance of this contract.
4.8 CLEANUP
The Contractor shall at all times keep the working area free from accumulation of waste material or rubbish. The contractor shall rake, sweep, blow or manually remove all debris including, but not limited to fallen palms fronds and branches, dead vegetation, fallen leaves, seed pods, paper, beverage cans, bottles and bagged yard debris from all vegetative and non-vegetation areas. Areas to include, but not limited to streets, sidewalks, median strips, playground, walkways, carports, and curbs. The work shall be performed regardless of presence of parked vehicles in the area. Upon completion of the work, the Contactor shall leave the premises in a clean and neat condition.
4.9 LOST AND FOUND
The Contractor shall ensure that all items of personal or monetary value found by the Contractor employees are returned to the Officer Of the Day (OOD), cell phone (808) 226-4170, or Junior Officer Of the Day (JOOD), cell phone (808) 221-1363, .
4.10 PERFORMANCE EVALUATION MEETINGS.
The Contractor shall meet with the Contracting Officer (KO) and/or Contracting Officer's Technical Representative (COR) as requested by the either parties.
