About this file

This document is a justification for limiting sources on a federal contract opportunity for an eyeForce treatment chair. The Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 1 has a requirement for an eyeForce stretcher chair from USKF that contains specific necessary features including e-Drive and steering guide functions, fold-down handles, lightweight design, programmable memory positions, and accessories such as an adjustable surgeon wrist wrest and flexible surgical drape support rod. These characteristics are peculiar to USKF's product and no other equivalent equipment is currently known to exist. The justification document supports limiting the source to USKF in accordance with FAR 8.405-6 on the basis that the chair contains unique features only available from this manufacturer.

VHAPG Part 808.405-6 Limiting Sources Attachment 1: Justification for Limiting Sources for Micro-Purchase Threshold up to the SAT

Effective Date: 02/01/2022 Page 2 of 2

In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order and the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order was not previously issued under sole-source or limited-sources procedures (FAR 8.405-6(a)(1)(i)(C)); or

Items peculiar to one manufacturer. An item peculiar to one manufacturer can be a particular brand name, product, or a feature of a product, peculiar to one manufacturer).

A brand name item, whether available on one or more schedule contracts, is an item peculiar to one manufacturer (FAR 8.405-6(b)).

6. Rationale/explanation to support cited authority:

The USKF Eyeforce stretcher chair has the following required characteristics:

“e-Drive” function (forward and backwards) which is motorized 5th wheel drive to travel longer distances (operating room to ambulatory procedure area for discharge, pathway is through 3 different doors and 5 corridors).

“Steering Guide” function can be activated from the head or the foot allowing entry and exit into the OR suite and out of the OR suite with ease.

Drive handles fold down and out of the way of surgical procedure.

Lightweight with four fully rotating double-run caster for easy rolling motion optimizing controlled handling of the chair by only a single person.

Individually programmable memo positions, reset button, and auto-run function.

Swiveling foot control for sensitive chair adjustment Low access height chair/stretcher for better patient positioning for surgery. SES extension system reduces access height by 50 millimeters. SES extension system also prevents patients from sliding while changing from a sitting to recumbent position.

Longitudinally adjustable headrest Battery lasts 35-40 surgery cycles with battery storage drawer allowing for quick emergency exchange of batteries intra-operative.

Easy recharging with easily accessible charging adapter and battery charger circuit board Accessories: Adjustable surgeon wrist wrest, flexible surgical drape support rod, padded swiveling upholstered side rails for patient safety, hand remote control bracket.

At this time it is unknown if any other equipment exists that contains the same features/characteristics. Brand name or equal will be accepted for this requirement.

7. Note that this determination satisfies the requirements of FAR 8.405-6. The requirement to prepare a written justification IAW FAR 6.303 does not apply to Federal Supply Schedules.
