About this file

This document is a Request for Limited Sources Justification for a federal contract to procure Pluvicto, a specialized radiopharmaceutical for treating metastatic castration-resistant prostate cancer. The Department of Veterans Affairs (VA) Phoenix Healthcare System seeks a delivery order from Novartis Pharmaceuticals Corporation for 30 doses per year at $33,229.14 per dose, with a total estimated contract value of $4,984,371.00, covering a base period and four option years from January 1, 2026, through December 31, 2030.

The justification highlights that Pluvicto is the first FDA-approved targeted radioligand therapy for this specific cancer treatment, with no therapeutically equivalent alternatives available in the United States. Novartis is the sole contractor listed on the VA National Acquisition Center (NAC) Pharmaceutical Catalog, and a special notice posted on SAM.gov in October 2025 received no responses from potential alternative manufacturers or distributors. The contract will be awarded under the Multiple-Award Schedule Program, with pricing verified as fair and reasonable by the VA NAC.

(4) Identify the Authority and Supporting Rationale (see below and if applicable, a demonstration of the proposed contractor’s unique qualifications to provide the required supply or service.

FAR 8.405-6(a)(1)(A): An urgent and compelling need exists and following the ordering procedures would result in unacceptable delays:

FAR 8.405-6(a)(1)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized.

Pluvicto is the first targeted radioligand therapy for the treatment of progressive, prostate-specific membrane antigen-positive (PSMA) positive metastatic castration-resistant prostate cancer. There are currently no other equivalent FDA approved treatments available in the United States.

VA National Acquisition Center (NAC) has contracted with the only authorized manufacturer of these medications, Novartis Pharmaceuticals Corporation under Federal Supply Schedule (FSS) number 36F79720D0180. There is only one vendor listed on the NAC Pharmaceutical Catalog portal that can provide this medication due to the proprietary and highly specialized nature of this medication.

FAR 8.405-6(a)(1)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The https://www.acquisition.gov/far/part-8#FAR_8_405_6

VHAPG Part 808.405-6 Limiting Sources Page 3 of 5 Revision: 01 Effective Date: 09/01/2024 Acquisition ID#: 36C262-26-AP-0681 original order must not have been previously issued under sole source or limited source procedures.

FAR 8.405-6(b): Items peculiar to one manufacturer:

A patent, copyright or proprietary data limits competition. The proprietary data is:

These are “direct replacements” parts/components for existing equipment.

The material/service must be compatible in all aspects (form, fit and function) with existing systems presently installed/performing.

(5) Describe Why You Believe the Order Represents the Best Value consistent with FAR 8.404(d) to aid the contracting officer in making this best value determination:

The NAC has already negotiated and determined the cost of the radiopharmaceutical is fair and reasonable. Also, it is the best interest of the government to establish a DO with the manufacturer, Novartis Pharmaceuticals Corporation, directly with the agreed upon cost of the proprietary and highly specialized radiopharmaceutical, Pluvicto, at $33,229.14 per 27mCi/ML.

Pricing was verified for accuracy by querying the VA NAC Pharmaceutical Catalog and directly with the manufacturer.

(6) Describe the Market Research Conducted among schedule holders and the results or a statement of the reason market research was not conducted.

The NAC pharmaceutical catalog, SAM.gov, and FPDS were used to research acquisition histories and methods during August 2025.

The VA NAC has contracted with the only authorized manufacturer of these medications, Novartis Pharmaceutical Corporation under FSS 36F79720D0180. Novartis Pharmaceutical Corporation is the only one contractor listed on the NAC Pharmaceutical Catalog for Pluvicto.

SAM.gov and FPDS returned the notice ID 36C24424Q0682, a special notice intent to sole source, posted by NCO 4 for Lebanon VA Medical Center to procure Pluvicto and Lutathera isotopes. FPDS revealed that the DO 36C24425F0144 against FSS 36F79719D0169 was awarded to Advanced Accelerator Applications USA, Inc. (ADACAP) on 12/19/2024 at the total cost of $1,934,973.04 for the ordering period 1/1/2025 – 12/31/2025 using FAR 8.406-6 procedure.

This finding concludes that there were no other interested contractors who had the capabilities to provide Pluvicto. Effective 2/1/2025, all distribution activities related to the radioligand therapy and radioligand imaging products, including Pluvicto, were transferred from ADACAP to its affiliate, Novartis Pharmaceutical Corporation, and Novartis became the distributor of Pluvicto. ADACAP remains the manufacturer.

VHAPG Part 808.405-6 Limiting Sources Page 4 of 5 Revision: 01 Effective Date: 09/01/2024 Acquisition ID#: 36C262-26-AP-0681

A special notice, an intent to sole source, was posted on SAM.gov October 10, 2025, to see if any other potential contractors have capabilities of manufacturing or distributing radiopharmaceutical equivalent to Pluvicto; however, no response was received.

Due to the specialized nature of these radiopharmaceuticals, Pluvicto, are not available from any other contractors. There is no therapeutically equivalent version of these radiopharmaceuticals available in the United States that are also FDA approved. Per VA NAC, Pluvicto is available from only one FSS contract holder with Novartis Pharmaceuticals Corporation under NAC FSS contract# 36F79720D0180. The pricing of Pluvicto at $33,229.14/EA are accurate per VA NAC pharmaceutical catalog.

(7) Any Other Facts Supporting the Justification:

Pluvicto are proprietary medications created by the pharmaceutical company, Novartis Pharmaceuticals Corporation.

(8) A Statement of the Actions, if any, the agency may take to remove or overcome any barriers that led to the restricted consideration before any subsequent acquisition for the supplies or services is made:

The Department of Veterans Affairs will continue to survey the marketplace for ‘equal to’ or better therapies that will offer veterans the most up to date care.

(9) Requirements Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge. I understand that processing of this limited sources justification restricts consideration of Federal Supply Schedule contractors to fewer than the number required by FAR Subpart 8.4.
