About this file

This Limited Sources Justification document is for the Department of Veterans Affairs (VA), Network Contracting Office 01, supporting VISN 01 Manchester VA Health Care System. The procurement is for Stryker Procuity and MV3 beds valued at $413,161.84, to be ordered against an existing Federal Supply Schedule (FSS) contract. The beds will replace existing Hill Rom and Stryker beds, with advanced features including integration with nurse call and Vocera systems, fall prevention technologies, and enhanced patient safety mechanisms.

The justification highlights critical fall-related statistics for the VA facility, noting a 5.83 falls per 1000 patient days rate, with over 90% of falls being unwitnessed and occurring in patient rooms. The new beds offer significant improvements, such as motion-activated under-bed lighting, built-in scales, a 550-pound capacity, side-to-side rotation, and compatibility with existing communication systems. The procurement leverages a VANEC Blanket Purchase Agreement and follows the Veterans First (Rule of Two) procurement approach, with the VA committed to continually monitoring emerging technologies for future fall prevention innovations.

(2) Description of Action: This acquisition is conducted under the authority of the Multiple-Award Schedule Program (41 U.S.C. 152(3) and 40 U.S.C. 501). The action being reviewed and approved will be awarded a new requirement as a FFP FSS BPA order.

Order against: FSS Contract Number: 36F79721D0021 VANEC BPA Number 36C24525A0024

Name of Proposed Contractor: Stryker Corporation Street Address: 3800 E Centre Ave City, State, Zip: Portage MI 49002 Phone: Amanda Blackburn, Amanda.Blackburn@stryker.com

(3) Description of Supplies or Services:

The estimated value of the proposed action is $413,161.84

VISN 01 needs Stryker Procuity and MV3 beds to augment existing beds, accessories, and surfaces/mattresses at the following VISN 01 locations: VISN 01 Manchester VA Health Care System at 718 Smyth Road, Manchester, NH, 03104-7007

(4) Identify the Authority and Supporting Rationale (see below and if applicable, a demonstration of the proposed contractor’s unique qualifications to provide the required supply or service.

FAR 8.405-6(a)(1)(A): An urgent and compelling need exists and following the ordering procedures would result in unacceptable delays:

FAR 8.405-6(a)(1)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized;

FAR 8.405-6(a)(1)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. FAR 8.405-6(b): Items peculiar to one manufacturer:

A patent, copyright or proprietary data limits competition. The proprietary data is:

These are “direct replacements” parts/components for existing equipment.

The material/service must be compatible in all aspects (form, fit, and function) with existing systems that are currently installed/performing. The new Procuity LEX beds will fully replace our existing Hill Rom and Stryker beds, integrating with our recently purchased nurse call system and Vocera hardware to enhance communication and safety, including immediate alerts when Veterans exit the bed. These beds will interface with the Vocera system and bed board to ensure accurate bed assignments, alarm management, and real-time staff notifications, reducing fall risks. They feature built-in accurate scales for monitoring fluid gain in Veterans with CHF exacerbation, a self-contained bed extender for Veterans over six feet, and a motion-activated under-bed light, which is especially important since our Veteran rooms only have lights that need manual activation—this feature benefits Veterans with dexterity or cognitive challenges. With a 550-pound capacity and side-to-side rotation, these beds improve mobility assistance, eliminate the need for bariatric beds, and provide a versatile, safe, and accommodating environment tailored to our Veterans’ diverse needs.

(5) Describe Why You Believe the Order Represents the Best Value consistent with FAR 8.404(d) to aid the contracting officer in making this best value determination:

GSA has already determined the prices of supplies and services under schedule contracts to be fair and reasonable. Therefore, ordering activities are not required to make a separate determination of fair and reasonable pricing. Since these beds are offered on a VA FSS schedule, the NAC determined price reasonableness, and the publicized prices are Firm-Fixed. Nevertheless, per IAW FAR 8.405-4, the Contracting Officer requested a price reduction.

(6) Describe the Market Research Conducted among schedule holders and the results or a statement of the reason market research was not conducted.

Market research was conducted according to the requirements at hand. During market research, a VANEC Blanket Purchase Agreement (BPA) was found. The VANEC Bed BPA is a multiple-award BPA awarded to Federal Supply Schedule (FSS) contract holders in accordance with (IAW) FAR 8.4. It is in the best interest of the VA to use this BPA. Veterans First (Rule of Two) was considered and applied in this BPA action.

(7) Any Other Facts Supporting the Justification:

Troubling #’s related to Falls . More than half of residents in a nursing home will fall each year 2(Mintz)

. Fall-related injuries (FRIs) are the most common source of injury among older adults and result in approximately 6% of total Medicare spending annually 3(Florence) . Falls are two to three times more frequent among residents living in long-term care (LTC) facilities than counterparts living in the community. 4(Harris) . According to a survey report at Musashino Red Cross Hospital, the behavior of patients and people in need of nursing care seems to show that fall accidents are most likely to occur before and after toileting, but about 60% of the fall accidents occurred at the bedside . Hip fractures are among the worst consequences of falls. Although relatively few falls result in hip fractures, for those that do that mortality rates for the year following a hip fracture range from 14% to 36%, with the higher estimates coming from residents of LTCFs 5(Carroll).

(8) A Statement of the Actions, if any, the agency may take to remove or overcome any barriers that led to the restricted consideration before any subsequent acquisition for the supplies or services is made:

Recognizing that advancements in bedside fall prevention have been limited until now, the Manchester, VA continues to direct the fall champion task force to continually monitor emerging technologies and best practices in healthcare safety. For future acquisitions, we will actively seek out the latest in technological innovations, such as beds with integrated fall prevention features, real-time alerts, and enhanced safety mechanisms, to support the goal of creating a fall-free zone for Veterans. Additionally, fostering partnerships with industry leaders and investing in pilot programs could help overcome previous barriers and ensure that procurement decisions are informed by cutting-edge solutions.
