About this file

This is a federal subcontracting compliance certification form for solicitation number 36C24126Q0467. Offerors must certify their compliance with 13 C.F.R. 125.6, which governs limitations on subcontracting for small business contracts. The form requires contractors to select one of two compliance paths: either certify that no subcontractors will be used and the prime contractor will perform 100% of the contract work, or certify that the prime contractor and similarly situated subcontractors will collectively perform at least 50% of the contract value, with each similarly situated subcontractor meeting the size standard for the assigned NAICS code and holding VIP certification.

For contractors selecting the second option, the form requires detailed information on each subcontractor including business name, DUNS number, description of work, and percentage cost of contract services. Contractors must provide totals for the percentage of contract value performed by the prime and by similarly situated subcontractors. The form includes a detailed warning regarding severe penalties for violations, which can include fines of up to $500,000 or the dollar amount spent in excess of permitted subcontracting levels, plus potential debarment. The Contracting Officer may request documentation such as invoices, subcontracts, or task value lists to verify compliance at any time during contract performance. Completed certifications must be signed and returned to the Contracting Officer via email.
