About this file

This document is a Limitations on Subcontracting Certification form related to the VA Boston Healthcare System's solicitation for Sterilizer Preventative Maintenance Services. The form requires contractors to certify their compliance with 13 C.F.R. 125.6, specifically indicating whether they will perform 100% of contract work or use similarly situated subcontractors who will receive at least 50% of the contract value. The certification includes a warning about severe penalties for violating subcontracting rules, with potential fines up to $500,000 or the excess amount spent on subcontractors.

The form mandates that contractors provide detailed information about any subcontractors, including business name, DUNS number, description of work, and percentage of contract cost. Contractors must understand that similarly situated subcontractors must meet the NAICS size standard and be certified in the Veteran-Owned Small Business (VIP) program. Contracting Officers may require contractors to demonstrate compliance through invoices, subcontract copies, or task value lists at any point during contract performance. The form must be signed and dated by the contractor and returned via email to the Contracting Officer.
