About this file

This document contains a certification form regarding compliance with limitations on subcontracting regulations. The form is for a solicitation to replace a 130-ton air cooled chiller at the Jamaica Plain Medical Center in Boston for the Department of Veterans Affairs. Offerors must certify either that no subcontractors will be used, or that the prime contractor and any similarly situated subcontractors will receive at least 50% of the contract value. If subcontractors are used, the prime must provide each subcontractor's business name, DUNS number, description of work, and percentage of contract cost. The form warns that penalties for noncompliance with limitations on subcontracting can include fines up to $500,000 or the amount spent in excess of permitted subcontracting levels. Contracting officers may check for compliance by requesting invoices or lists of task values performed at any time during contract performance.
