About this file

The document is a Minority Business Enterprise (MBE) Form for the City of Fort Myers, Florida, specifically related to the Downtown Basin Repairs Rebid (ITB 25-004). The form is designed to document contractors' good faith efforts in meeting the city's MBE goal of 15%, requiring respondents to provide detailed information about prime and subcontractors, including contact methods, communication records, and potential barriers to MBE participation.

The form includes six steps for documenting MBE involvement: prime contractor information, MBE contractor details, record of transmittal letters, telephone/in-person visit documentation, handling of incomplete quotations, and explanation of non-utilized MBE firms. The form aims to promote transparency and accountability in the procurement process by requiring contractors to demonstrate active efforts to include minority-owned businesses in the project, with space to detail outreach attempts, communication methods, and reasons for not utilizing specific MBE contractors.
