About this file

The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, related to a municipal contract opportunity. The form requires bidders to certify whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212, Sections 212-1 through 212-11) and comply with living wage requirements for eligible employees. Bidders must indicate their status by selecting either that they are a covered employer who will pay the required living wage or that they are exempt due to specific conditions such as employing fewer than 25 eligible employees, having an annual contract value less than $25,000, or being a nonprofit organization with specific tax-exempt status.

The form provides three potential exemption categories: employers with less than 25 eligible employees, contracts with an annual value under $25,000, and charitable foundations or nonprofit agencies exempt from federal income taxation under Section 501 of the Internal Revenue Code. The form must be completed, signed, and submitted with the bid or RFP, and failure to do so may result in bid rejection. The certification requires the bidder's authorized representative to verify the accuracy of their claimed status, with space provided for the bidder's signature, printed name, and date, along with fields for the specific bid/RFP number and name.
