About this file

This is a Living Wage Certification Form required by the Town of Manchester for contractors bidding on municipal contracts that may be subject to the Manchester Living Wage Ordinance (Chapter 212, Sections 212-1 through 212-11 of the Manchester Code of Ordinances). The form requires bidders to certify whether they are a "Covered Employer" subject to living wage requirements or qualify for an exemption, with failure to complete and return the form potentially resulting in bid rejection. The document does not specify particular products, services, response dates, or contract terms as it appears to be a standard certification form used across multiple procurement opportunities.

The form establishes three primary exemption criteria for contractors who are not covered employers: companies employing fewer than twenty-five eligible employees, contracts with annual values less than $25,000, and charitable foundations, trusts, or nonprofit agencies exempt from federal income taxation under Section 501 of the Internal Revenue Code. Covered employers must commit to paying the required living wage to eligible employees and complying with all ordinance requirements throughout the contract term. The certification requires an authorized representative to sign and date the form, providing their name, company name, and the specific bid/RFP number and name for which they are submitting.
