About this file

This is an LGBTQ Applicant Data Form related to a micro purchase opportunity issued by a Pennsylvania municipality for MacBook computers, webcams, and surge protectors. The procurement will be awarded as a whole or by line item, at the City's discretion, to the lowest responsive and responsible bidder. Sellers must submit quotes for all items to be eligible for award consideration. Delivery and service provision must not commence until official notification is received from the department. This is structured as a requirements contract, meaning the City will purchase needed quantities of the specified items as demands arise during the contract period.

The form serves as a certification document for LGBTQ-owned business applicants participating in this procurement opportunity. Specific pricing, set-aside provisions, contract duration, and renewal terms are not detailed in this particular document. Vendors should reference the full solicitation terms and conditions, particularly Part B regarding requirements contract provisions, for complete information on pricing structures, payment terms, and other contractual obligations.

The data will be used to identify the number of LGBTQ businesses currently doing business with the City. It will also assist in efforts to include LGBTQ certified businesses into the City’s vendor database.

By submitting this form you are agreeing that the City may, in its discretion, use the data submitted for other projects and may cause it to become publicly available.
