About this file

This is a Request for Proposal (RFP) issued by Lafayette County, Wisconsin d/b/a Lafayette Manor for Construction Manager as Constructor (CMc) services for a new long-term care and short-term rehabilitation facility. The project involves pre-construction and construction management services for a 70,723 square foot facility featuring approximately 50 skilled nursing beds, 18 assisted living beds, 12 memory care beds, skilled nursing and sub-acute rehabilitation services, therapy spaces, and administrative offices. The selected CMc will not self-perform any work; all subcontractors will be chosen by the County through competitive bidding using the lowest responsible bidder process. Proposals were due September 20, 2021, with interviews scheduled for September 22 through October 5, 2021, and CM selection estimated by October 18, 2021. The preliminary project schedule includes design from October 2021 through October 2022 and construction from October 2022 through March 2024, with occupancy targeted for March 2024. The project is subject to obtaining adequate USDA financing and closing the purchase of the property at the intersection of WI State Highway 23 and WI County Highway K in Darlington, Wisconsin.

The CMc fee structure is based on a Cost of the Work plus Fee with Guaranteed Maximum Price (GMP) model under AIA Document A133-2019. The fixed fee for pre-construction services is $20,000, with the construction phase fee to be proposed as a percentage of the actual Cost of the Work, not to exceed the GMP. Payment and performance bonds are required at 100% of the Contract Sum plus net increases from Change Orders. Insurance requirements include commercial general liability of $5,000,000 per occurrence and $5,000,000 aggregate, automobile liability of $1,000,000 per accident, workers' compensation at statutory limits with employers' liability of $500,000, and pollution liability of $2,500,000. The project will utilize Lafayette County funds and USDA financing, with project delivery in a collaborative environment alongside the design team. Additionally, a separate Lafayette Hospital Project may be designed and constructed simultaneously on the same property; the CMc may submit a combined proposal demonstrating savings if selected for both projects, though one, both, or neither project may proceed depending on financing availability and property purchase completion.
