About this file

This document is Addendum 1 for KEYS Bid 08-25, issued by the KEYS Utility Board in Florida for local lodging during emergency situations. The bid establishes a three-year base contract with the option to extend for two additional one-year terms upon mutual agreement. The addendum provides clarification on bidding requirements, specifically addressing questions about contract duration, third-party proposal submissions, and required certification forms. The document outlines detailed instructions for proposals submitted by partnerships and corporations, including requirements for signatures, authorizations, and potential requests for additional documentation.

The addendum addresses specific concerns from potential bidders regarding certification forms, particularly items 6 and 7 related to debarment and contract certifications. KEYS confirms that these are required forms found in the Terms and Conditions section of the bid specifications, and any exceptions should be noted separately in the proposal for review. The document emphasizes that bidders should carefully review all required forms and provide comprehensive information, with the understanding that all listed items are potentially applicable to the bid submission.
