About this file

This document is a U.S. Department of Agriculture (USDA) Farm Service Agency Container Loading Observation Worksheet (KC-41 form) used to document container loading processes for a vessel loading observation contract. The form is designed to record details about container loading, including categories of container damage (damaged and not loaded, damaged and loaded, damaged and removed, repaired and loaded), with space for tracking up to 10 containers. The form includes a certification section where the observer acknowledges potential legal consequences for providing false information, with references to specific U.S. Code sections related to fraud and false statements. The reverse side of the document contains privacy act statements, public burden information, and a nondiscrimination statement outlining USDA's policy against discrimination in its programs and activities.
