About this file
This document outlines proposal requirements for KASTOW projects in Kentucky. Bidders must submit a single PDF document with consecutively numbered pages that address site-specific conditions rather than generic templates. The proposal must follow a strict organizational structure including a cover page, transmittal letter, bidders' experience qualifications, prior projects summary, contractors' committed equipment list, proposed work plan, Davis-Bacon Act requirements, subcontractor identification form, cost proposal with bidder's response forms, annual required affidavit for bidders and contractors, and a signed 5% bid bond. Proposals that deviate from this format or include unnecessary pages such as blank sheets or section dividers may be deemed non-responsive.
The document emphasizes that all proposal content must be Kentucky-specific and tailored to site conditions to receive favorable evaluation grades. Bidders are instructed to exclude generic workplans and equipment lists, as these will be graded accordingly for their lack of specificity. The strict adherence to document organization, page numbering, and inclusion of mandatory components is critical for proposal acceptance, as failure to comply with these requirements may result in the proposal being rejected as non-responsive.
