About this file

This is a Justification and Approval (J&A) document for a sole source award under the Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside authority.

The Department of Veterans Affairs, Veterans Integrated Service Networks (VISN) 07, is awarding a 3-month bridge IDIQ contract to Hometown Veterans Medical, LLC for home oxygen and ventilator services at the VA Augusta Health Care System. The contract period runs from June 1, 2026 to September 30, 2026, with a price list containing multiple line items for oxygen-related supplies and services at specified unit prices and estimated monthly and 4-month quantities. The acquisition is being sole-sourced to prevent service interruption, as the current contract (36C24726D0016) expires May 31, 2026 and the new long-term procurement is pending an Agency Protest decision. The incumbent contractor has performed satisfactorily with no complaints and possesses the necessary qualifications to provide medical oxygen cylinders, accessories, delivery, setup, preventive maintenance, and repair services to veteran patients.

The statutory authority for this other-than-full-and-open-competition award is 41 USC §3304(a)(5) and 38 U.S.C §8127(c), as implemented by RFO 6.103-5. The acquisition falls under VAAR 819.7008 (SDVOSB sole source awards). Market research identified 582 certified SDVOSBs under NAICS 621610, and SAM.GOV searches identified several competing SDVOSBs including TransOx, Inc., Panakeia LLC, Eagle Home Medical Corp, and Veterans First Healthcare. The Contracting Officer certified the proposed price as fair and reasonable based on comparison to the current contract, IGCE, and market survey data. The J&A was signed by Leonard Robinson, Contract Officer at RPO-East, Network Contracting Officer 07, on May 15, 2026, with required approval from one level above the Contracting Officer.

Request for SDVOSB/VOSB Sole Source Justification Format IAW VAAR 819.7008/9

VHAPG Part 819.7008/7009 Sole Source Awards to SDVOSB/VOSB Page 3 of 5 Revision: 05 Effective Date: 12/15/2025 new long-term requirement pending the Agency Protest decision. Hometown Veterans Medical, LLC, the incumbent, provides services and has the qualifications to meet the demands of setting up and delivering home oxygen services, providing education and operating instructions directly to veteran recipients on the safe use of home oxygen machines. Hometown Veterans Medical, LLC has been performing satisfactorily with no complaints for the VA Augusta Health Care System. In addition, it is in the Government's best interest to sole-source this acquisition to Hometown Veterans Medical, LLC to prevent a break in services (Continuation of Services).

6. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable, including whether a notice was or will be publicized as required by RFO 6.301 and, if not, which exemption under RFO 5.101(b)(1) applies: Based on the current contract status for home oxygen services, there is a need to ensure that direct patient care is not interrupted. An interruption in services could cause harm or death to patients. The current home oxygen contract is expiring, and the new long-term procurement will require additional time due to the Agency Protest. As a result, the requirement is to be awarded to Hometown Veterans Medical, LLC, as a sole source, to prevent a service interruption and to allow time to award the new long-term contract.

7. Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: The cost to the Government was deemed fair and reasonable based on the current contract comparison to the current contract, the IGCE, and the market survey comparing prices to similar supplies and services. The current monthly cost ranges between . The proposed price for services remains unchanged.
