About this file
This is a Justification for Non-Competitive Procurement (JNCP) document for services below the Simplified Acquisition Threshold (SAT) from the Naval Hospital Guam's Neonatal Stabilization Unit/Mother Baby Unit. The procurement covers two critical neonatal care services: Neonatal Total Parenteral Nutrition (neo-TPN) and methemoglobin testing to support a new 3-bed Neonatal Stabilization Unit providing 24/7 care for critical neonates for up to 10 days.
The document justifies sole-source procurement from Guam Regional Medical City (GRMC) as the only capable provider on Guam, citing their unique capabilities in neo-TPN formulation/compounding and specialized methemoglobin testing equipment. The justification cites geographical constraints, time-sensitivity of services, and GRMC's exclusive ability to provide custom-compounded neo-TPN solutions and methemoglobin testing for critically ill infants. Market research conducted from February to April 2024 confirmed GRMC as the sole provider with these capabilities on the island. The document is dated January 7, 2025 and includes a plan to explore future partnerships with local/regional hospitals to establish additional healthcare facilities that could provide these services and increase competition.

4) Reason for Non-Competitive Procurement
An urgent and compelling need exists, and following the procedures would result in unacceptable delay
Unique or highly specialized requirement/Only one source reasonable available Exclusive Licensing Agreements Logical Follow-on to previously competed work The material or service must be compatible in all aspects (form, fit, and function) with existing systems presently installed (identify systems and why other products are not compatible) Patent, copyright, or proprietary data limits competition (identify specifics and corresponding level of Government rights – if any) Direct replacement parts/components for existing equipment and similar parts/components will not meet Government requirements End to End Engineering (For example, Command X supports engineering and/or development efforts for Program of Records and supplies are required from OEM.)
Testing (For example, Command X supports laboratory testing that must replicate shipboard/command environment without deviation.)
Software (Either license and maintenance renewals with compatibility and proprietary coding and data warranty requirements.)
5) Provide rationale and justification regarding the above selection.
Neonatal Total Parenteral Nutrition (neo-TPN)
• Formulation Precision: Neonatal TPN requires highly specialized formulations that are tailored to the unique nutritional needs of premature or critically ill infants. These formulations often involve specific concentrations for critically ill neonates.
• Custom Compounding: Neonatal TPN solutions must often be custom compounded to address the specific needs of each infant based on weight, gestational age, and medical condition. This level of customization and expertise in neonatal care is solely offered at GRMC on the Guam island.
• Supply and Distribution Constraints: The island’s geographical location has limited the availability of suppliers who can meet the stringent requirements of neonatal TPN. Shipping logistics, and distance prevent alternative suppliers from providing deliveries of the specialized formulations required for critically ill neonatal patients.
Methemoglobin Testing:
• Specialized Equipment or Test Kits: Methemoglobin testing requires specific equipment, reagents, and test kits that are only available at Guam Regional Medical City (GRMC).
• Geographical Constraints: The methemoglobin testing must be conducted on the Guam island, and geographical isolation has limited the availability of multiple laboratory test centers. The Naval Hospital Guam (NHG) has identified GRMC has the only reliable testing center within reasonable transportation distance or logistical capability to carry out the test.
• Time Sensitivity: Methemoglobin testing and results are needed in urgent medical situations for critically ill infants and GRMC is the sole hospital that can quickly provide the necessary services to meet immediate healthcare needs and provide quality service.
GRMC is the sole hospital on the island of Guam that can support neonatal TPN and methemoglobin testing for neonatal critical care patients.
6) Describe the Market Research conducted.
We contacted local hospitals, clinics, and laboratory test centers on Guam island and inquired about the parenteral nutrition solutions specifically (neo-TPN) and methemoglobin testing. Despite inquiries with multiple healthcare facilities from February to April 2024, it was found that only GRMC could provide neo-TPN and methemoglobin testing.
