About this file
This is a Justification and Approval (J&A) document for other than full and open competition from the U.S. Naval Hospital Guam, Defense Health Agency, for the sole source procurement of 48 cases of Alcon Vision LLC's Hi-Infinity Phaco-Emulsification Cataract Packs (P/N AS13120-18).
The J&A justifies a purchase order valued at $132,888.00 with delivery required by January 31, 2025. Alcon Vision LLC is identified as the sole manufacturer of these customized ophthalmic supplies, with market research confirming no alternative sources through UNICOR, AbilityOne, or GSA Advantage. While Alcon has GSA Contract Number 36F79722D0040 for ophthalmic surgical equipment, it does not cover these specific Cataract Packs. The justification cites that failure to procure these packs from the manufacturer could jeopardize patient care and result in canceled medical procedures. The Contracting Officer determined pricing to be fair and reasonable based on the Independent Government Cost Estimate and historical pricing.
