About this file
This document is a Justification for Other Than Full and Open Competition for the Department of Veterans Affairs (VA), specifically for a sole source contract with Medicom Technologies, Inc. The contract is for Medicom ImageX Assistant Edition Software, including subscription licenses, support services, customization, maintenance, and training for the VA's GenISIS platform. The 12-month base contract includes implementation of a standalone ImageX instance behind the VA firewall, with four 12-month option periods for continued access and maintenance.
The VA determined Medicom is the only vendor capable of meeting their requirements, based on market research showing the software enables secure, bi-directional medical image and report exchange across VA medical facilities. Medicom ImageX is already implemented in 15 VA Healthcare Systems across 11 states and provides unique functionality that cannot be replicated by competitors like Ambra and Nuance. The contract is valued at $58,500 and will be awarded on 2025-06-27, with the primary goal of maintaining a peer-to-peer network for electronic medical image sharing that supports Veterans' diagnostic imaging needs both within VA facilities and with external healthcare providers.

Medicom ImageX CONTROL NUMBER VA-25-00028386 images. Medicom will allow Imaging Services to search the commercial imaging providers located within the VISNs, locate prior medical images and reports, and retrieve them to VA where they can be inducted to VA systems behind the GenISIS firewall. Medicom ImageX operates solely within the GenISIS platform, ensuring data remains within the VA network and does not connect to external cloud services.
Medicom ImageX are already in place across 15 Veterans Administration Health Care Systems and VA Medical Centers across 11 states for clinical image data exchange. If VA were to switch vendors from Medicom, VA facilities would have to remove existing services from complete sites, and re-engage the legal teams of all sites to interface with a new vendor’s system, which is estimated to take multiple years to rebuild as well as additional costs that would not be recovered through competition.
VA requires a system which enables electronic, bi-directional exchange of radiology imaging exams and reports with commercial healthcare facilities providing services to VA patients. Moreover, the Mission Act has enabled Veterans to seek diagnostic imaging services outside of VA. If VA loses Medicom’s ImageX and support services, digital image sharing would be unavailable. VA would have to use compact disks (CDs) and digital video disks (DVDs) that are hand-carried or mailed to and from VA facilities at the completion of a diagnostic test, ultimately creating bottlenecks and delays in the continuity of Veteran care. Delays in archiving images can also cause misplaced CDs or DVDs, repeated examinations, and “unreadable” images that are incompatible with some softwares in VA. All adverse factors add to unnecessary health risks of Veterans and additional costs to VA. Therefore, a peer-to-peer network that provides VA the ability to electronically exchange data within the community is required.
Medicom is the only vendor able to provide timesaving solutions to fit the unique needs of VA. Based on the above information, only Medicom meets all the Government’s requirements and is the only capable source.
6. Efforts to Obtain Competition: Market research was conducted, details of which are in the market research section of this document. This effort did not yield any additional sources that can meet the Government’s requirements.
There is no competition anticipated for this acquisition. Additionally, the proposed action will be synopsized on the Contracting Opportunities Page in accordance with FAR 5.201. Any proposals that are received shall be evaluated.
7. Actions to Increase Competition: The Government will continue to conduct market research to ascertain if there are changes in the marketplace that would enable future actions to be competed.
8. Market Research: The Government’s technical experts conducted market research in December 2024 for competing products to Medicom Imagex Assistant Edition. Products offered by Ambra and Nuance were found to provide similar functionality. However, the competing products do not provide all the necessary functionality required by the VA. Specifically, the other products require the use of a SaaS solution and cannot be installed behind the GenISIS firewalls.
On April 30, 2024, Medicom Technologies provided a letter stating Medicom is the only source for Medicom software, support services, maintenance and training.
Medicom does not have any authorized distributor or resellers. Based on these market research efforts, the Government Technical expert has determined that only Medicom Technologies can meet the Government’s needs. As such, there is not an expectation of competition even in a limited capacity.
