DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C244-21-AP-2707

1. Contracting Activity: Department of Veterans Affairs, VISN 04, Altoona VA Medical Center, 2907 Pleasant Valley Blvd., Altoona, PA 16602. PR: 503-21-2-225-0130

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Altoona VAMC has a BRAND NAME requirement for IAC Acoustics Audiometric Sound Booths, Natus Medical Audio Equipment, Delivery, Installation and Training for the Indiana Community Based Outpatient Clinic (CBOC) AND Huntingdon CBOC.

The Huntingdon CBOC requires one brand name (1) IAC Acoustics 40a-5 Single Walled Audiometric Sound Booth, Additional Duplex Outlets, one (1) Door Upgraded, one (1) Sound Booth Installation and Delivery, and one (1) Enhancement Package for 1AC 40a-5 Booth.

The Indiana CBOC requires one brand name (1) IAC Acoustics 40a-5 Single Walled audiometric Sound Booth, additional Duplex Outlets, Door Upgraded to 36”, Sound Booth Installation and Delivery, and Enhancement Package for IA 40a-5 Booth.

The Indiana and Huntingdon CBOCs each require one (1) brand name Natus Medical of the following (for a total of two of each item): GN Otometrics, Astera Clinical Audiometer without HAD, Astera High Frequency Option, including Hi-Freq headsets, Audiometer Control Panel, Amplifier, Sound Field, 95dB, Installation and Training, Otometrics Otocam 300, Zodiac SA Diagnostic Middle Ear Analyzers, including Classic Probe, Contralateral Insert Phone, and Built in Printer, Zodiac Installation and Training, and AURICAL (PMM & C&S) without Hi-Pro, and Installation and Training for Aurical. The intended use is to provide audiology services in both communities without causing undue travel and delay in care for the patients served in those areas. The estimated amount of this requirement is $121,821.18

Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: VA Medical Center, Altoona currently utilizes the Industrial Acoustic Audiology equipment and integration systems. The requested brand name equipment is currently being used and all staff including the audiologist are familiar with the equipment. The manufacturer provides calibration for all equipment which includes five (5) different sets. This is an additional add-on for current equipment to be used within the department. The audiology equipment connects to current OTO Suite Immittance Software module, which is part of the VA Gold Image used throughout the VA https://www.acquisition.gov/?q=/browse/far/13

Medical Center. Although there are other vendors capable of providing the equipment, it would not be feasible nor cost effective to use another vendor. This requirement is a complex requirement needing equipment, installation, and training with the same manufacturer. The current local vendor is familiar with the locations involved and can provide the necessary installation and training as well as being able to provide any necessary service and/or warranty work for the equipment during the equipment completion and lifecycle.

3. Description of market research conducted and results or statement why it was not conducted:

Market Research was conducted via Federal Procurement Data System, Google, GSA e-library, and SAM. Searched VIP using NAICS 339112 and keyword Audio Booth Audiometer which resulted in Search VIP Searched 13 SDVOSB/VOSB’s. The search via NAC MedSurg Catalog did not include any audio booths that fit the requirements (all booths listed were ‘mini-booths’). Sources Sought Notice was posted to FBO and yielded 1 SDVOSB distributor, 3 Small businesses and 1 large business. Per the manufacturer the equipment is sold via exclusive PA distributors, however, manufacture letter noted that there are other vendors that may also distribute the equipment. The requirement will be competed on the open market using a 100% small business set aside.

4. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106- 1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
