3. Description of Services or Supplies: This requirement is required because in 2017 the Joint Commission established a new requirement that all critical access hospitals must meet the requirements of the Antimicrobial Stewardship as established by the Centers for Disease Control (CDC). The Infectious Disease Society of America (IDSA) created the Antimicrobial Stewardship Center of Excellence designation to highlight those institutions meeting/exceeding this requirement. Brooke Army Medical Center, JBSA – Ft Sam Houston, Texas has earned this designation.

5. Reasons for Authority Cited:
Due to requirements established by the Centers for Disease Control (CDC) requiring all critical access hospitals must meet the requirements and be established with the Antimicrobial Stewardship Center of Excellence to be recognized with the CDC. Designation of this caliber is prestigious for Brooke Army Medical Center and adheres to the Joint Commission requirement that all critical access hospitals meet requirements for antimicrobial stewardship at established by the CDC. Traditionally, professional institutions/universities (i.e., other-than-small businesses) are the only entities capable of fulfilling the requirement of accreditations for hospitals. The Infectious Disease physicians and is the only entity currently offering the Center of Excellence of designation in accordance with meeting centers for Disease Control )CDC criteria.
6. Publicizing Contract Actions: For FAR Part 13 actions, the public display and synopsis requirements of FAR 5.101 and FAR 5.203 do not apply due to the $ threshold, apply and will be followed, or do not apply due to an exception at .

7. Market Research: Research was done by using public search engines such as Google and Bing. The search was done by using key words "membership and license" and the majority of results found were for the Infectious Disease Society of America and information established by the Centers for Disease Control (CDC). A Dynamic Small Business Search was also conducted using key words "bone and marrow" and "transplant" with NAICS code 813920, Professional Organizations; the results found do not indicate the capability to fulfill the Government's requirements. Upon final approvals, this J&A will be posted to https://beta.sam.gov, the Government Point of Entry.
8. Other Facts: The government will continue to monitor the market for additional sources, but at this time there is only one source that can provide these services due to requirements established by the CDC. Past requirements of this nature have been solicited and posted on Government point of entry without obtaining responses from industry. The government will continue to seek new resources that will satisfy this requirement through continued market research to determine if there are new technologies and companies that can provide these services.

11. Contracting Officer Certification/Approval: I hereby determine that the anticipated cost to the Government for this contract action will be fair and reasonable. Historical procurement data from similar contracts and market research will be used to determine that the price is fair and reasonable. Certified cost or pricing data is not required per FAR 15.403-1(b)(3).
I certify that this justification is accurate and complete to the best of my knowledge and belief, and that completing this purchase as a sole source is in the best interest of the Government.
Additionally, I approve this justification to limit competition subject to the availability of fund and provided that the services or supplies herein described have otherwise been authorized for acquisition.
