About this file

This document is a Past Performance Rating Form (Attachment J.P-6), designed to evaluate a contractor's performance across five key criteria: Technical Quality of Service, Cost Control, Schedule/Timeliness, Management/Business Relations, and Small Business Subcontracting. The form provides a standardized mechanism for rating contractor performance using an adjectival rating scale ranging from Exceptional to Unsatisfactory for each evaluation criterion.

The rating form requires detailed input from both the contractor (in the Project Identification section) and the rater (typically a Contracting Officer, Representative, or Government employee). For each performance criterion, the rater must select an appropriate rating and can provide additional comments, particularly for Marginal or Unsatisfactory ratings. The form is specifically structured for Federal Prime Contract Awards and includes specific considerations for small business participation, with emphasis on meeting statutory goals, utilization of various small business categories, and compliance with Federal Acquisition Regulation (FAR) requirements.
