1. Contracting Activity: Department of Veterans Affairs, VISN 04, VA Butler Healthcare, 353 N. Duffy

Road, Butler, PA 16001. Purchase Request (2237) Number: 529-20-3-094-0391.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Zoll X-Series Manual Monitor Defibrillator provides the ability to monitor, pace, shock, record, and transmit and print vital reports for Veterans experiencing a cardiac-related emergency. This new, brand name request for the Zoll Defibrillator allows VA Butler s Emergency Medical Technicians (EMT) to transmit Electrocardiographs (EKG) to the receiving facility, Butler Memorial Hospital and Butler Ambulance Service. The Zoll Defibrillator is the only compatible equipment the VA Butler Healthcare, Butler Memorial Hospital and Butler Ambulance Service can utilize.

The estimated value of the proposed action is

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

This new, brand name request is peculiar to one manufacturer, Zoll Medical Corporation. The Butler VA Healthcare is required to purchase the Zoll Defibrillator due to its interoperability with Butler s local medical receiving facilities. By purchasing this equipment, it will ensure there are no delays in transferring Veteran care during cardiac related emergencies. Also, the leads for the Zoll Defibrillator are unique and if another brand was purchased the receiving medical care sites would need to remove the leads attached by VA medical providers and replace with leads compatible with Zoll. The unique leads offered by Zoll is a special wire that delivers energy from a pacemaker or implantable cardioverter defibrillator (ICD) to the heart muscle. If Butler VA Healthcare purchased a different brand this would directly impact and jeopardize Veteran care.

4. Description of market research conducted and results or statement why it was not conducted:
