About this file
This is a Modification & Exception Form for the State of Idaho's In Vitro Diagnostic (IVD) Test System Lease procurement (ITB1215). The document pertains to modifications and exceptions being requested or documented for the IVD test system leasing contract opportunity. The form serves as an official modification instrument through which changes to the original procurement terms, specifications, or requirements may be communicated to prospective bidders. The original procurement was posted on February 9, 2026, with a proposal submission deadline of March 5, 2026, providing a 24-day response window for interested vendors to prepare and submit their proposals.
The specific modifications and exceptions detailed in this form would establish any changes to the original procurement parameters, including potential adjustments to technical specifications, evaluation criteria, pricing requirements, timeline adjustments, or other contractual terms. Without access to the completed details within the form itself, the exact nature of the modifications cannot be determined; however, such forms typically address clarifications requested during pre-bid conferences, corrections to the original solicitation, or exceptions to standard procurement procedures. Interested vendors should obtain the complete modification and exception form along with the full Request for Proposals to ensure full compliance with all current procurement requirements prior to submission of their responses by the March 5, 2026 deadline.

ATTACHMENT 2 - MODIFICATION AND EXCEPTION FORM
Instructions: Complete this form and include with your ITB submittal if you are proposing modifications or exceptions to any of the requirements, terms, or conditions included in the ITB, including any documents incorporated by reference (Standard Contract Terms and Conditions). See ITB Section 3 for a full explanation of the process surrounding Bidder-proposed modifications and exceptions.
Bidders must specifically address any and all proposed modifications and exceptions. Blanket requests to negotiate requirements, terms, or conditions will not be considered. Bidders must provide an explanation as to why the requirement, term, or condition should be considered non-material. Bidder must also provide a reason for the proposed modification or alternative language, specifically addressing the issues itemized in ITB Section 3.
The determination of materiality will be made at the State’s sole discretion. Non-material modifications or exceptions may be negotiated with the apparent successful Bidder, at the discretion of the State, and as otherwise provided in ITB Section 3.
