About this file

This is a Responsibility Inquiry form (Attachment F) associated with ITB S-25700-00006196 for a Three-Dome Railroad Tank Car Training Simulator procurement by the State of Oregon's Department of Administrative Services. The form serves as a pre-award responsibility determination tool that bidders must complete to demonstrate their capability and qualifications to perform the contract. The document does not specify quantities, response dates, site visits, or bidder conference details, as it functions as a post-bid evaluation instrument rather than a solicitation announcement. Bidders are required to submit the completed form along with any additional documentation requested by the Agency, which may include financial statements, bonding capacity information, and performance history records.

The form requires bidders to affirm their possession of necessary financial, material, equipment, facility, and personnel resources to meet contractual obligations. Key responsibility criteria assessed include: contract performance history over the past five years regarding schedule and budget adherence; criminal or civil charges within three years related to public contracts, antitrust violations, or fraud; contract terminations for default or litigation within three years; outstanding judgments and financial distress status; bankruptcy filings within three years; required licenses, insurance, and legal authorization to conduct business in Oregon; and possession of a current Pay Equity Certificate from the Department of Administrative Services. Failure to promptly provide requested information or demonstrate responsibility may result in an Agency finding of non-responsibility and bid rejection. An authorized representative must certify under signature that all responses are complete, accurate, and not misleading.
