1.0 BACKGROUND INFORMATION

It is our understanding that Modular Office Building, Storage Building, and Office Building, located at 2971

US Highway 1 North, Louisville, GA 30434 have been vacated. As vacated, these structures are scheduled to be demolished. To comply with current regulations as defined below, these structures must be thoroughly inspected for asbestos-containing materials (ACM) prior to demolition.

The United States Environmental Protection Agency’s (EPA) National Emissions Standards for Hazardous

Air Pollutants (NESHAP) and Georgia Environmental Protection Division (EPD) Regulations require that

ACM be appropriately addressed prior to any renovation or demolition activity. The United States

Occupational Safety and Health Administration’s (OSHA) Occupational Exposure to Asbestos Standards, including the General Industry Standard [29 CFR 1910. 1001] and the Construction Industry Standard

[29 CFR 1926.1101] require the determination of the presence, location, and quantity of ACM or presumed asbestos-containing materials (PACM) at the work site.

To support the planned demolition of the structures, PenPoint, LLC was requested by Mr. Jim Clute, State Facilities Manager, Georgia Department of Transportation (GDOT) to provide asbestos survey efforts to identify ACM potentially impacted by the planned demolition.

2.0 SCOPE OF SERVICES

The scope of our asbestos survey efforts included:

1. Visual survey of the interior and exterior of the structure and the identification of suspect asbestos-containing materials.

2. Sample collection of suspect materials and laboratory analysis of the samples to determine the presence (or absence) of asbestos.

3. Report preparation

3.0 ASBESTOS-CONTAINING MATERIALS SURVEY

3.1 Visual Survey and Observations

Donna Folkner, an AHERA-accredited Asbestos Building Inspector (Number 15658) performed the asbestos survey efforts on June 30, 2016. Mr. Randal Brett escorted the survey team throughout the facilities and provided usage, construction history, and historical information. The survey began at the

Storage Building (Structure 2). The visual survey began with a walk around the exterior of the structure, and then proceeded to the interior of the structure. The structure, approximately 800 sq ft, was of concrete masonry unit/brick façade exterior walls, with a frame roof system including plywood decking and covered with asphalt shingles. The ceiling finish was painted drywall in the center section and exposed structure in the storage sections. The exposed block walls were painted on the interior. The

PenPoint Project Number 1784 Page 2 floor was the exposed concrete slab. Although no documentation was available relating to the construction of the structure, it is assumed to have been constructed in the 1960’s.

The next structure to be surveyed was the brick Office Building (Structure 3). Similar in construction to the Storage Building and assumed to be constructed in the 1960’s as well, this building had served as the original area office. The structure, approximately 1300 sq ft, was of concrete masonry unit/brick façade exterior walls with a frame roof system including plywood decking and covered with asphalt shingles.

The ceiling finish was drywall coated with a textured ceiling finish. The exposed block walls on the perimeter were painted on the interior. Office and support spaces are divided by wood stud partition walls finished with drywall. Floor surfaces were finished with 9” x 9” resilient floor tile and associated adhesives.

After the site efforts were completed at the Office Building, the survey team went to the Modular Storage

Building (Structure 1). Again, the visual survey began with a walk around the exterior of the structure, and then proceeded to the interior of the structure. The Modular Storage Building, a commercial office modular structure of approximately 3000 sq ft, is approximately 28 years old according to Mr. Brett and was in use until about 8 months ago when the new office was completed. Typical of modular construction, the interior finishes were pre-finished gypsum board ceilings, paneled or pre-finished gypsum board wall finished, and 12” x 12” floor tiles.

The survey efforts included identification of suspect asbestos-containing materials as well as the quantification of those suspect materials. Suspect materials are those construction materials that have historically contained asbestos. A homogeneous material area is one which contains suspect that seem by texture, color, and wear to be uniform, and that appear to have been applied and used during the same time period. Collectively, within the homogeneous material areas, the following suspect materials were identified:

Multilayers of asphalt shingles – Structures 1, 2, and 3

Floor tiles – Structures 2 and 3

Drywall – Structures 1, 2, and 3

Ceiling Tiles – Structure 1

Ceiling Texture – Structure 3

PenPoint Project Number 1784 Page 3

3.2 Sample Collection and Analysis

Following the visual identification of suspect materials, a sampling strategy was developed to comply with regulatory protocols and sample locations were selected. Samples were collected from areas of readily accessible suspect materials. Because NESHAPs requires a “thorough inspection” prior to demolition, our survey efforts included limited destructive sampling and sample locations were not repaired after collection.

Twenty-three samples of suspect materials were collected and delivered to Call Analytical Laboratory, a

NVLAP accredited laboratory. The laboratory certification certificate is included as an attachment. Sample analysis was performed utilizing the EPA method of Polarized Light Microscopy (PLM) coupled with dispersion staining, “Method for the Determination of Asbestos in Bulk Building Materials” EPA/600/R-

93/116. A PLM Report Summary is provided with this report that identifies materials sampled, describes corresponding sample locations, and analytical results.

3.3 Conclusions

Asbestos-containing materials were not identified by PenPoint’s survey efforts in Structure 1 – Modular

Office Structure.

Asbestos-containing drywall joint compound was identified in the ceiling of the office area of the

Structure 2 – Storage Building.

Structure 2

Structure 1

Structure 3

In Structure 3 – Office Building, asbestos-containing floor tile and mastic, ceiling texture, and drywall joint compound in the ceiling and interior partition walls were identified by PLM analysis.

In the following Attachments section, a summary asbestos inspection report describing asbestos-containing materials and quantities of those asbestos-containing materials, bulk sample summary with analytical results, and photo log are included.

General Notes:

Survey Protocol. The asbestos survey and sample collection efforts utilized to identify and delineate the extent of asbestos-containing materials within the structures on these parcels meet existing Environmental Protection Agency (EPA) NESHAPs regulatory requirements. The corresponding bulk sampling of suspect materials complies with current OSHA (AHERA/ASHARA) requirements for the identification of asbestos-containing materials in buildings, (i.e., referenced AHERA protocol). Materials historically known to have used asbestos in their manufacture, such as, Transite panels, cement asbestos board and pipes, and black roof mastic were not sampled and are assumed to contain asbestos.

Definition of ACM. Current NESHAPs regulations define asbestos-containing materials (ACM) as those materials containing 1 percent asbestos. NESHAPs notification and disposal requirements, enforced in the State of Georgia by the Georgia EPD, are

“triggered” by materials containing 1 percent asbestos. Though existing OSHA regulations define ACM as those materials containing 1 percent asbestos, the regulations also apply to all materials identified to contain asbestos, no matter how minute the quantity, (i.e., 1 percent).

Wallboard and Joint Compound. Existing EPA regulations address wallboard and joint compound as a composite system, and allow composite sampling to determine the asbestos content of the combined wall system. However, OSHA does not allow composite sampling of wallboard and joint compound; Rather, OSHA’s asbestos regulations require individual sampling of each wall system component. While existing NESHAPs regulatory requirements for notification and disposal do not apply for composite wallboard systems containing 1 percent asbestos, OSHA’s Asbestos Construction Industry Standard (49CFR5816.5801) does apply. OSHA’s regulatory requirements include but are not limited to worker training; engineering controls and work practices, communication of hazards, and waste disposal do apply to wallboard systems with any identified asbestos content.

Limitations. At the time of the asbestos survey and subsequent reporting efforts, it is not known if the subject structures will be relocated for reuse or demolished. Therefore, given the possible relocation/reuse of structures, the damage to existing building components and finishes was minimized and destructive access was not utilized to access potentially concealed suspect materials.

In the event the structures are demolished rather than relocated, suspect materials previously concealed which are uncovered by the demolition efforts (such as Transite panels, cement asbestos flues, pipe insulation, etc.) should be assumed to contain asbestos or properly sampled by an AHERA-accredited Asbestos Inspector to verify otherwise. Provide, as necessary, appropriate notifications or amendments to prior GA EPD notification regarding the increase in asbestos-containing material.

1.0 BACKGROUND INFORMATION

It is our understanding that the Louisville Area Office modular building has been vacated and abandoned due to excessive mold growth resulting from faulty and unrepairable construction. As currently vacated and abandoned, this structure is scheduled to be demolished. To comply with current regulations as defined below, this structure must be thoroughly inspected for asbestos-containing materials (ACM) prior to demolition.

The United States Environmental Protection Agency’s (EPA) National Emissions Standards for Hazardous Air Pollutants (NESHAP) and Georgia Environmental Protection Division (EPD) Regulations require that ACM be appropriately addressed prior to any renovation or demolition activity. The United States Occupational Safety and Health Administration’s (OSHA) Occupational Exposure to Asbestos Standards, including the General Industry Standard [29 CFR 1910. 1001] and the Construction Industry Standard [29 CFR 1926.1101] require the determination of the presence, location, and quantity of ACM or presumed asbestos-containing materials (PACM) at the work site.

To support the planned demolition of the structure, PenPoint, LLC was requested by Mr.

Donna Folkner, an AHERA-accredited Asbestos Building Inspector (Certificate No. 18361, expiration 3/08/2022) performed the asbestos survey efforts on May 29, 2021. The visual survey began with a walk around the exterior of the structure, and then proceeded to the interior of the structure. The Louisville Area Office modular building is a modular office structure of approximately 4000 sq ft constructed in 2015.

Interior finishes include resilient tiled floors, vinyl base molding, prefinished gypsum wall panels with joint covers, and suspended acoustical tile ceilings. Fiberglass batt insulation has been installed directly on top of the ceiling tiles in the attic space and lines the exterior wall cavities. The roof is an elastomeric membrane.

2971 US Highway 1 North, Louisville, GA 30434 June 4, 2021 PenPoint Project Number 2275 Page 2

The survey efforts included identification of suspect asbestos-containing materials as well as the quantification of those suspect materials. Suspect materials are those construction materials that have historically contained asbestos.
