About this file

This is a compliance affidavit document required by the State of Michigan for all bids, proposals, and qualification statements submitted in response to state contracting opportunities, including the Select Exterior Metal Painting Project. Effective April 1, 2013, all respondents must certify their compliance with Michigan's Iran Economic Sanctions Act, 2012 P.A. 517, and confirm their eligibility to submit bids under state law. The affidavit requires respondents to certify under civil penalty for false certification that they are not an "Iran linked business" as defined in the Act.

Respondents must sign and submit this certification at the time of bid submittal. The document requires the bidder's signature, title, company name, and date of submission. Failure to provide this certification or providing false information subjects the respondent to civil penalties under Michigan law.
