About this file
This is a template form for conducting intra-service Section 7 biological evaluations under the Endangered Species Act, designed for use by U.S. Fish and Wildlife Service (USFWS) offices in Region 3. The form is initiated by a USFWS Project Officer (originating person) and completed collaboratively with the grant applicant, who is instructed to provide project-specific information because they possess the most detailed knowledge of the project area and potential impacts on listed species. Grant applicants are required to generate a threatened and endangered species list for their project area using the online IPAC tool at https://ecos.fws.gov/ipac/ and submit it with the completed form.
The form requires documentation of the service program, geographic location (state and county), all federally-listed, proposed, and candidate species or critical habitat that may occur within the project area, and a detailed project description including conservation elements, maps, and site photos. Applicants must provide a comprehensive description of how the proposed project will affect listed species and critical habitats, followed by a formal determination marked as either "No Effect," "May Affect but Not Likely to Adversely Affect," or "May Affect and Likely to Adversely Affect." The reviewing Ecological Services Field Office then evaluates the submission and indicates concurrence, nonconcurrence, or requirements for formal or conference consultation. This biological evaluation is a prerequisite for federal grant awards under the Great Lakes Restoration Initiative and other USFWS-administered grant programs that may affect threatened or endangered species or their designated critical habitats.

Originating Person: Nathan Evans Telephone Number: 612-286-4212 Date Submitted:
Project Title:
This evaluation form will be submitted by the USFWS Project Officer to the appropriate USFWS Ecological Services Office. Your assistance is requested to complete as many of the items on this form as possible because as the grant applicant, you are the most familiar with your project, project area, and potential effects on listed species. Given this is a consultation between USFWS Offices (thus an “Intra-Service Section 7”), the USFWS Project Officer/Originating Person will review (and potentially modify) any information you provide and will complete any items that you do not. To generate a threatened and endangered species list for your project area, please utilize: https://ecos.fws.gov/ipac/. If you generate a species list, please provide that with this form.
I. Service Program and Geographic Area or Station Name: Fisheries
II. Project Location. Include state and county:
III. Species/Critical Habitat. List all federally-listed, proposed and candidate species or designated or proposed critical habitat that may occur within the project area:
IV. Project Description. Describe proposed project or action, including all conservation elements. If referencing other documents, prepare an executive summary. Include map and photos of site, if possible. Attach additional pages as needed:
V. Determination of Effects A. Description of Effects. Describe how the action(s) will affect the species and critical habitats listed in item III. This includes a full description of your rationale for the Section 7 determination made in the next section.
B. Determination. Indicate the anticipated effects of the proposed project on species and critical habitats listed in item III by marking each applicable “effect” and the species or critical habitat to which it pertains (or attach a list indicating the same).
Mark all that apply:
( ) No Effect. This determination is appropriate when the proposed project will not directly or indirectly affect (neither negatively nor beneficially) individuals of listed/proposed/candidate species or designated/proposed critical habitat of such species. No concurrence from Ecological Services Field Offices (ESFO) required.
List species/critical habitat in this determination:
· Critical habitats
( ) May Affect but Not Likely to Adversely Affect. This determination is appropriate when the proposed project is likely to cause insignificant, discountable, or wholly beneficial effects to individuals and designated critical habitat. Concurrence from ESFO required.
List species/critical habitat in this determination:
· Critical habitats
( ) May Affect and Likely to Adversely Affect. This determination is appropriate when the proposed project is likely to adversely affect individuals of listed species or designated critical habitat of such species. Concurrence from ESFO required.
List species/critical habitat in this determination:
Signature (Supervisor at originating station):
Date:
Reviewing Ecological Services Office Evaluation (check all that apply):
( ) Concurrence.
