About this file

This document outlines insurance coverage requirements for contractors providing services to the Department of Technology, Management & Budget (DTMB) at State of Michigan locations. Contractors must maintain Commercial General Liability insurance with minimum coverage limits of $2,000,000 for general aggregate and products/completed operations aggregate, $1,000,000 for personal and advertising injury, and $1,000,000 per occurrence. The State of Michigan, its departments, divisions, agencies, offices, commissions, officers, employees, and agents must be listed as additional insureds on all liability certificates. Contractors using motor vehicles to provide services must also maintain vehicle liability insurance covering owned, hired, and non-owned vehicles as required by law, with the State similarly named as additional insured. All insurance policies must be primary and non-contributing to any comparable coverage carried by the State, and contractors must provide evidence of waiver of subrogation by their insurance companies.

Subcontractors must comply with the same insurance requirements as the primary contractor, either by purchasing separate coverage or being included under the contractor's existing policies. Successful bidders are required to submit certificates of insurance on the standard accord form to DTMB-Financial Services, Operations Division, Delegated Procurement Unit at the Cass Building, 2nd Floor, 320 S. Walnut Street, Lansing, Michigan 48913, prior to purchase order issuance. Failure to provide the necessary certificates of insurance before purchase order issuance will result in award being given to the next competitive bidder. All certificates must be prepared and submitted directly by the insurance provider and must clearly identify DTMB-Financial Services-Operations Division, Delegated Procurement Unit, and the service location.
