About this file

This is an Insurance Checklist document issued by the City of West Sacramento establishing certificate of insurance requirements for all standard city contracts. The document specifies mandatory insurance coverage for contractors and vendors, including Commercial General Liability of $1,000,000 per occurrence/$2,000,000 aggregate, Commercial Automobile Liability of $1,000,000 per accident, Workers' Compensation per statutory requirements, and Professional Liability (Errors & Omissions) of $1,000,000 per claim if applicable. All policies except Workers' Compensation and Professional Liability must name the City as an additional insured, with required endorsements including Additional Insured Endorsement equivalent to CG 2010 0714, Primary and Non-Contributory Endorsement, and Separation of Insureds endorsement. Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City, its officers, agents, and employees, or a Workers' Compensation Waiver for self-employed individuals with no employees. All certificates must provide thirty days' written notice to the Certificate Holder prior to cancellation or material change of any insurance. The Certificate Holder is identified as the City of West Sacramento officers, officials, employees, agents, and volunteers, with the city address listed as 1110 West Capitol Avenue, West Sacramento, CA 95691.

This insurance checklist applies to the 2026 Pavement Rehabilitation Project, a design consulting contract for roadway improvements across ten identified street segments in Yolo County, California. The project scope includes determining appropriate pavement treatment options and developing plans, specifications, and estimates. The Request for Proposals was released October 13, 2025, with proposals due November 3, 2025, at 2:00 pm, and a non-mandatory pre-proposal meeting scheduled for October 21, 2025. The contract term is three years with options to extend for up to two additional one-year terms. The project is locally funded through Measure O with no specific set-aside designations. The comprehensive nature of the work, covering approximately 132,931 square yards of roadways with Pavement Condition Index ratings between 35 and 51, combined with extensive insurance and contractual requirements, indicates a significant municipal infrastructure investment.
