About this file
This is a Preventive Maintenance (PM) Inspection Report for a medical bulk oxygen system. The inspection was conducted on October 14, 2025, at the George H. Obrien VA Medical Center in Big Spring, Texas, by technician Ben Hill. The system consists of a 1500-gallon bulk oxygen tank (Model E-240400, manufactured by Linde) with a Maximum Allowable Working Pressure (MAWP) of 250 psig, equipped with pressure safety valves, burst disks, and dual ambient vaporizers (Thermax/Chart TMF048A-HF-SG). The system includes a Generant regulator (Model 2GDR-1000B-V-BM) set to deliver pressure at 55 psig with a final line pressure safety valve set at 75 psig, along with cylinder reserve supply consisting of 9 x 51-inch cylinders with 2100 psig pressure.
All 15 inspection categories were evaluated and passed, including overall site condition, vessel condition, equipment condition, and medical system requirements. Critical safety measures verified include adequate site access and ventilation, vessel anchoring and concrete pad integrity, pressure relief device installation, piping leak checks, and compliance with NFPA 55 standards. The medical alarm system was tested and confirmed functional with set points for main vessel low level (30 inH₂O), reserve-in-use (47 inH₂O), and reserve low pressure (1600 psig). Notably, during the inspection, technician Ben Hill replaced pigtails and discovered six isolated empty cylinders on the manifold, which were replaced with full cylinders available on site, and two additional missing cylinders that were also replaced. No other equipment issues were identified, and the system was verified to automatically switch to reserve supply with no unauthorized modifications.
