PERFORMANCE WORK

STATEMENT

FOR

Infrared Inspection of Electrical Manholes

1. DESCRIPTION OF SERVICES:

1.1. Scope: The Contractor shall provide all labor, tools, equipment, hoses, gauges, test equipment, hearing protection, eye protection including prescription safety glasses, materials, supplies and parts, transportation and other incidentals necessary to travel to Hanscom AFB, MA, to clean out and provide thermo graphic survey (infrared scan) of identified electrical manholes throughout the base. The Contractor shall be responsible for ensuring all services and parts provided under this contract are in accordance with the terms and conditions outlined herein, and that they conform to all manufacturers’ equipment specifications and this Performance Work Statement (PWS).

1.2. Requested Service: This project is intended to clean all manholes in “Appendix B”, pump water out of all manholes in “Appendix B” and provide a thermo graphic survey (infra-red scan) of identified electrical manholes in “Appendix B”. The data and results of this survey will be used to identify deficiencies in the underground electric utility system, in order to prevent unscheduled power outages in accordance with all applicable National, Air Force, State and local building codes and regulations. This work will consist of supplying/furnishing all labor, materials, tools, equipment, reports, and expenses necessary to accomplish the work described below in compliance with the Commonwealth of Massachusetts Building Code and Massachusetts General Law requirements, state and federal codes, base standards and manufacturer’s instructions. Where there is a conflict between nationally recognized codes and local codes, the most stringent shall apply.

1.2.1. Technicians shall be compliant with the requirements set forth by the Code of Federal Regulations, Title 29, Part 1910 for Confined Space Entry Certified (See Para 1.3 Confined Space).

1.2.2. Contractor shall fully clean out all manholes prior to scanning. Cleaning shall include, but not be limited to, the removal of any and all standing water, sludge and debris.

Discharge shall not cause an erosion situation.

1.2.3. The Contractor shall be responsible for the disposal of all debris and material removed from the manholes. Debris shall be transported, at the Contractor’s expense, from Hanscom AFB to any landfill that is permitted by Mass DEP to accept solid waste. The Contractor is permitted to place a container on base at a site pre-approved by the government representative to aid in the collection of manhole debris.

1.2.4. The Contractor shall notify the Government Representative, two weeks in advance, of any work that will impede high traffic areas and intersections. This information will be used for base wide notifications.

1.2.5. The Contractor shall provide a flag person(s) for traffic control in compliance with the standards set forth by the Manuel of Uniform Traffic Control Devices (MUTCD), or as deemed necessary by the Government Representative.

1.2.5.1. IR scans shall be performed on all electrical splices, elbows, racks, and connections to include neutrals and grounds as required by each unique manhole arrangement.

Temperature(s) shall be recorded in Celsius (C°) for each connection scanned.

1.2.5.2. The Contractor shall inspect all cables and ducting for physical, electrical and mechanical condition.

1.2.5.3. The Contractor shall remove all necessary manhole covers prior to thermo graphic inspection.

1.2.6. The Contractor shall inspect distribution systems with imaging equipment capable of detecting a minimum temperature difference of one (1) degree Celsius at thirty (30) degrees Celsius.

1.2.7. Equipment shall detect emitted radiation and convert detected radiation to visual signal.

1.2.8. Thermo graphic surveys shall be performed during periods of maximum possible loading, but not less than forty percent (40%) of rated load of the electrical equipment being inspected. (NFPA 70B-1990, paragraph 18-16.5).

1.2.9. The Government Representative shall be notified verbally, immediately and in writing within a week, of finding a situation that is prone to failure.

1.2.10. The Contractor shall provide electronic (CD) report with findings of existing conditions and any recommendations for corrective maintenance priorities.

1.2.11. The Contractor shall provide resolutions of any technical problems that arise during the service being performed.

1.2.12. Should any change in scope be required, it shall be coordinated with the CE Government Representative and CO and approved as a modification prior to performing the change in work.

1.2.13. The Contractor shall provide periodic updates to the CE Government Representative.

Changes to schedules or schedule slippages with justification must be reported at regular intervals.

1.2.14. Contractor shall comply with AFH 32-1282, Field Guide for Inspection, Evaluation, and Maintenance Criteria for Electrical Transformers.

1.2.15. The thermo images taken from the top of the manhole are not acceptable.

1.2.16. The Contractor shall complete “Appendix A” including butterfly drawing depicting all sides of the manhole with field notes along with racks, elbows, and splices for each electrical manhole surveyed.

1.2.17. IR scans shall be performed on electrical connections as required by each unique manhole arrangement. Temperature(s) shall be recorded for each connection scanned.

Manholes and elbows shall be marked upon completion. It is expected and intended that all physical cleaning and inspection work shall be completed prior to the accumulation of snowfall or other adverse conditions sufficient to impact safety and accessibility as determined by the Government Representative. Any portion of the work schedule suspended due to inclement conditions shall not resume until such conditions have subsided as determined by the Government Representative.

1.2.18. Reports: The Contractor shall provide two (2) hard copies and one (1) electronic report (CD) with findings of existing condition, specific to each manhole, and any recommendations for corrective maintenance priorities. Reports shall include pictures of all elbows, the modules they rest on (racks), splices, grounding connections, and any hazardous condition that may exist.

A calibration report for the IR scanner shall be provided with the final report. All reports are due within thirty (30) days of completion of the inspection of the final manhole as verified by the Government Representative.

1.2.18.1. Thermography Survey Report shall be categorized to include the following:

a. Category 1. Temperature differences of one (1) degree Celsius to three (3) degrees Celsius indicate possible deficiency and warrant investigation only.

b. Category 2: Temperature differences of four (4) degrees Celsius to fifteen (15) degrees Celsius indicate possible deficiency; repair at a planned date in the future.

c. Category 3: Temperature differences of sixteen (16) degrees Celsius and above indicate major deficiency; immediate repair required.

1.2.23.2. The contractor shall fill out Hanscom manhole inspection diagram for each manhole inspected “Appendix A”.

1.3. Confined Space: Qualifications: A qualified person shall be trained and knowledgeable of the construction and operation of equipment and tools required to implement the PWS, and shall be trained to recognize and avoid the electrical hazards that might be present with respect to that equipment or work method. Such person shall be familiar with the proper use of the special Precautionary techniques; personal protective equipment including arc flash suit; insulating and shielding materials; and insulated tools and test equipment. Such person shall be experienced of working within the limited approach boundary of exposed energized electrical conductors and circuit parts operating at 15kV or more. Qualifications shall also include regular experience installing 15kV + gear conductors and regular experience installing splices on 15kV+ distribution.

1.3.1 The infrared thermographer shall: Have sufficient knowledge of the components, construction and theory of electrical or mechanical equipment, or both, to understand the observed patterns of radiation, be knowledgeable of, and comply with, the safety practices and rules of the end user, and 5.1.3 Use an infrared imaging system with imaging or radiometric capabilities, or both, sufficient to meet the examination requirements.

1.3.2. The infrared thermographer shall develop an inventory list of the equipment to be examined in a logical, efficient and safe route through the facility. The infrared thermographer shall perform a calibration test(s), before each quantitative infrared examination, to ensure that all temperature-measuring equipment is within the manufacturers ‘standard specifications for accuracy. The infrared thermographer shall perform examinations when the environmental and physical conditions, such as solar gain, wind, surface and atmospheric moisture and heat transfer are favorable to gathering accurate data.

1.3.3. The infrared thermographer shall at least have a minimum of Level 1 (One) training certification.

1.3.4. The contractor shall be present with the subcontractors ALL THE TIMES during any work on base.

1.3.5. The infrared thermographer shall carry his/her qualification card ALL THE TIME during his/her work.

1.3.6. The infrared thermographer shall perform examinations when the environmental and physical conditions, such as solar gain, wind, surface and atmospheric moisture and heat transfer are favorable to gathering accurate data.

1.4. Quality Assurance: According to the contract’s inspection clause, the Government will evaluate the contractor’s performance under this contract. The COR is a representative of the Contracting Officer (CO) and will participate in the administration of this contract. Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the CO. All services to be performed by the Contractor shall be subject to review by the CO or COR. The Contractor shall provide an answer to a submitted Corrective Action Report (CAR) and Customer Complaint Form from the COR within one (1) working day from receipt. The Contractor shall provide written response addressing the root cause, corrective action and preventive actions (s).

1.5. Continuation of Essential Services during Crisis: This requirement has been determined not to be essential, and does not require continued support during a crisis as defined in DFARS Clause 252.237-7023.

1.6. Hours of Operation: The Contractor shall perform work Monday through Friday under this contract during the following hours: 07:00 a.m. until 3:30 p.m., excluding Federal Holidays. Work required to be performed outside of these hours shall only be done after approval from the Contracting Officer.

1.7.1 Hanscom recognizes the following holidays:

New Year’s Day - 1 January Martin Luther King Day - 3rd Monday in January Washington’s Birthday - 3rd Monday in Feb Memorial Day - last Monday in May Independence Day - 4 July

Labor Day - 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day - 11 November

Thanksgiving Day - 4th Thursday in November Christmas Day - 25 December

If these holidays fall on Saturday, the preceding Friday shall be observed. If these holidays fall on Sunday, the following Monday shall be observed. If a holiday falls on a scheduled service day, the Contractor shall be responsible for rescheduling services for the first day either preceding or following the holiday.

1.8. Contractor Personnel: The Contractor shall not employ persons on this contract if such employee is identified to the Contractor as a potential threat to the health, safety, security and general well-being or operational mission of the installation and its population. The Contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the Contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to the DoD 5500.7-R Joint Ethics Regulations (JER). In addition, the Contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities. The Contractor shall be prohibited from employing off-duty COR’s who are surveilling any contracts/subcontracts awarded to the Contractor. All contract employees, depending on particular condition, occasion or place, or job assignment shall require professional, suitable attire. All Contractor employees shall conduct themselves in a professional, courteous manner. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor employees in performance of this PWS shall be the responsibility and prerogative of the Contractor; however, the Contractor shall comply with the requirements set forth in this PWS and Hanscom AFB regulations concerning conduct of employees as referenced herein. The Contractor shall employ and maintain a technically trained and experienced work force.

1.9. Environmental Protection and Safety: The Contractor shall be knowledgeable of, and comply with, all applicable federal, state and local laws, regulations and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws or regulation changes during the term of this contract. The Contractor shall follow manufacturers’ guidelines and professional recommendations for control of humidity, temperature, cleanliness and materials handling (including hazardous materials).

1.9.1. The Contractor is solely responsible for compliance with the Occupational Safety and Health Act of 1974 (OSHA) and the protection of their employees and all sub-contractor The Contractor is solely responsible for compliance with the Occupational Safety and Health Act of 1974 (OSHA) , which includes and the protection of their employees and all sub-contractor employees. Due to the inherent dangers pertaining to this contract, the Contractor shall submit a written contractor safety plan (see US Army Corps of Engineer Safety and Health Requirements Manual EM 385-1-1) prior to performing work. The safety plan shall include and identify the details of the Contractor's safety organization, responsibilities, method of program implementation, and how corrective action is to be done. In performing work under this contract on a Government installation, the Contractor shall report any accidents involving Air Force personnel injuries, illnesses, or property/equipment damage, or Contractor personnel injuries occurring on the job or an accident resulting in death, injury, occupational disease, or adverse environmental impact to the Contracting Officer within five days of each incident. All provisions in AFFARS Clause 5352.223-9001, Health and Safety on Government Installations apply to this contract.

1.10. Hazardous Material (HAZMAT): The Contractor shall not store chemicals/HAZMAT on Hanscom AFB. All chemicals/HAZMAT shall be approved prior to being transported on base for use by any Contractor. The Contractor shall prepare and submit to the COR, ten (10) calendar days after contract award date, one (1) each, Chemical/Hazardous Material Request Authorization Form (Air Force Form 3952) and Material Safety Data Sheets (SDS) for each chemical, material and substance proposed to be used in performance of this PWS. The COR will forward the completed AF Form 3952(s) and SDS to 66 ABG/CEIE for approval. The Contractor shall not transport or use any chemical, material or substance on Hanscom AFB before written approval is provided by 66 ABG/CEIE. Contractor shall maintain copies of SDS(s) for all chemicals. The SDS(s) shall correspond with the manufacturers of the chemicals on hand. Manufacturer, material, and SDS shall match. NOTE: SDS refers to manufacturer SDS. Once material is approved for use, contractor shall contact base Environmental Management (EM) (66 ABG/CEIE) and provide a list of all material. 66 ABG/CEIE shall determine any additional material requirements based on the hazard code of the material.

1.11. Hazardous Waste: Unless there is evidence of contamination by a spill or some other means, material may be disposed at any landfill that is permitted by Mass DEP to accept solid waste. Contaminated manholes shall be evaluated in accordance with the Hazardous Waste Regulations, 310 CMR 30.000, and handled as Hazardous Waste if appropriate. Hazardous Waste Manifests shall be signed only by a designated official from 66 ABG/CEIE. The Contractor shall coordinate with 66 ABG/CEIE prior to disposing of any Hazardous Waste.

1.12. Discharges: Any discharges into industrial waste treatment plant (IWTP) or sanitary sewer shall be done in accordance with Hanscom Industrial Waste Water Discharge Permit as instructed by 66 ABG/CEAN. The Contractor shall maintain records of written authorization issued by 66 ABG/CEAN for discharges into IWTP and sanitary sewer.

1.13. Spills: Upon occurrence of any spills that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident.

When possible, the Contractor shall use a base telephone to receive help from Hanscom AFB

911. If using a cell phone, Contractor shall request Hanscom AFB 911. The Contractor shall report all environmental violations to the COR. The Contractor shall submit a typed or written report about the incident within four (4) clock hours. The Contractor shall provide five (5) copies of the written report. The Contractor shall provide one copy to the CO, two copies to the COR, a copy for Unit Environment Coordinator (UEC) and a copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information.

1.) Name and contact number of person reporting the spill 2.) Date and time of incident 3.) Location and source of spill 4.) Substance or pollutant spilled 5.) Amount spilled and the rate of discharge 6.) Any damages or injuries involved 7.) Extent of the area impacted 8.) Potential hazards 9.) Action taken 10.) Organizations contacted 11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equipment) and whether 911 was called and which agencies responded.

1.14. Housekeeping: The Contractor shall keep the work areas clean and neat in accordance with Federal, State, and Industry Standards, Occupational, Safety and Health Administration (OSHA), fire and safety standards and this PWS.

1.15. Tool Control and Accountability: While performing contract services in or around Maintenance Production Shops including Ramp areas, the contractor shall comply with AFI 21-101_AFMC Sup 1, 21-101, Chapter 10 (all paragraphs). The Contractor or Contractor’s personnel shall inventory the container or bag of tools at the beginning of each work day and at the end of each work day. Contractor personnel shall immediately notify the COR of any tools that are missing or lost from the work area or container (i.e. tool or job boxer tool bag).

The COR will report to appropriate Government personnel of any missing contractor’s tool(s) not located within one hour after notification of lost or missing tools.

1.16. Privately Owned Motor Vehicles (POV): Contractor personnel shall comply with the directives pertaining to operation of POVs on Hanscom AFB per AFI 31-204 (in its entirety), Motor Vehicle Traffic supervision.

1.17. Visitor Pass: The Contractor shall comply with requirement and procedures IAW FAR 5353.242-9000, Contractor Access to Air Force Installations. A list of names shall be required to gain access to the installation Passes shall be distributed at the visitor’s center at the Sartain (former Vandenberg) gate. Workers shall have proof of identification as well as a valid driver’s license, registration and insurance if bringing a vehicle onto base. Contractor employees shall have identification that readily identifies them as contractors. During the course of the contract, certain areas/sites may be restricted or closed to Contractor personnel. The Contractor shall comply with all requirements for entry. In addition, unscheduled gate closures may occur at any time and personnel entering or exiting the installation may experience delays.

1.18. Foreign National(s): The Contractor shall notify the COR before sending a Foreign National representative to perform services. Special rules apply for foreign national(s) visiting a US Government facility.

1.19. Health and Safety on Government Installation:

In performing work under this contract on a Government installation, the Contractor shall comply with federal regulations, state and local laws regulations regarding occupational safety and health and the specific health and safety requirements established by this contract and Appendix C. The Contractor shall notify the COR of any injuries that occur to Contractor personnel while on the installation. The Contractor shall notify the COR of damage to Government property or equipment during the execution of the contract

1.20. Emergency Procedures: Contractor employees shall follow the direction of Government employees in regards to emergency evacuation procedures, i.e. fire, tornado, active shooter and bomb threats. It is the Contractors’ responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with Government personnel in all fire and tornado drills, and at no additional cost to the government.

SECTION 2

Performance Assessment

The Contractor shall be aware that the absence of any contract requirement from the Service Summary does not detract from its enforceability nor limit the rights or remedies of the Government under any other provision of the contract. The Contractor shall ensure the following service summary requirements are accomplished.

SECTION 3

GOVERNMENT-FURNISHED PROPERTY AND SERVICES

3.1. Government Furnished Property (GFP): Government Furnished Property is not applicable in the performance of this contract.

3.2. Reserved.

3.3. Government Furnished Services: Government Furnished Services are applicable during the performance of this contract.

3.3.1. Security Police: The Government will provide general on base Security Forces service. The Security Forces’ telephone number is 911 for emergencies, for crime in progress 781-225- 5613 and 781-225-5613 for non-emergency calls.

3.3.2. Fire and Emergency Services: The Government will provide general on-base fire protection services. The Contractor shall comply with fire prevention rules and regulations enforced by the Hanscom AFB Fire Prevention Branch. Call 911 for emergencies. For routine fire prevention matters and/or calls to dispatch, call 781-225-3330.

Note: Calling 911 from a cell phone will be received by off base emergency 911 systems, causing a delay in response time by emergency personnel. When possible use a base telephone to receive on base help.

3.3.3. Utilities: The Government will furnish utility services. These utilities include natural gas, electricity, water and sewer for the sole purpose of accomplishing this contract. If the Contractor requires additional utility services, it shall be the Contractor’s responsibility to provide such services. Contractor shall use due care and diligence in efforts to conserve utilities in order to reduce utility costs.

3.3.4. Refuse Collection: The Government will not provide refuse collection services. Government dumpsters shall not be used for collection of Contractor debris.

3.3.5. Workspace: The Government will provide adequate workspace around the equipment area.
