About this file
This document is a Single Source Justification form for a simplified acquisition procedure by the Department of Veterans Affairs (VA), VISN 16, Little Rock Medical Center. The VA intends to purchase Illumina Next Generation Sequencing Supplies for genetic testing and sequencing, specifically consumable supplies for an Illumina MiSeq genetic sequencer currently in use at the facility. The justification details that Illumina Inc. is the sole source for these highly specialized supplies, as they do not authorize other companies to sell the consumable items and no alternative manufacturers produce compatible products. The acquisition is valued under the Simplified Acquisition Threshold of $250,000, with the purchase request number 598-25-4-5701-0124, and has an effective date of 11/15/2024. The contracting officer, Bennett W. Smith, has certified the justification as accurate and complete in accordance with Federal Acquisition Regulation (FAR) 13.106-1(b).
