About this file

This is a Request for Quote (RFQ) for providing noon meals (sub sandwiches) to applicants processing through the Salt Lake City Military Entrance Processing Station (MEPS). The contract is for a base period of 01 February 2026 through 31 January 2027, with two additional one-year option periods through 31 January 2029. The government guarantees to procure $15,000 of noon meals, with a fixed quantity of 8,082 meals per year. The solicitation is set aside for Women-Owned Small Business (WOSB) and is classified under NAICS code 722310 with a size standard of $47,000,000. The contract will be awarded to the lowest-priced technically acceptable quote, evaluated based on a health inspection report and past performance criteria. Vendors must submit quotes by 10:00 AM Eastern Time on 23 December 2025, with pricing to be firm for 90 calendar days.

Evaluation factors include a state/local health inspection report for the food preparation facility (requiring a score of "A" or "B" or "pass") and past performance assessment. Past performance will be rated as acceptable or unacceptable, considering relevancy (contracts valued at minimum $25,000) and recency (within previous 36 months). The contract will be issued by the Mission and Installation Contracting Command (MICC) Fort Knox, with delivery to the Salt Lake City MEPS location. Vendors must comply with various federal acquisition regulations and provide documentation including health inspection reports and potential subcontractor agreements.

II. Quote Submission Requirements:

a) Vendors shall ensure all required documents are attached with their quotation, only documents in .pdf format will be accepted. Attachments submitted in a format other than .pdf and documents accessible only through hyperlinks will be considered non-compliant.

The proposed food service preparation facility is to be used for the duration of the awarded contract unless a change is approved by the Contracting Officer. The health inspection shall be completed less than 12 months from the date quote submissions are due. The facility must have received a score of "A" or "B"; 85 or above or "pass", if pass/fail inspection system is used. The score must be clearly annotated on the report. Failure to provide an acceptable health inspection report will render the quote non-compliant.

c) One copy of any required subcontractor agreement, fully executed by both the prime contractor and the subcontractor (if any portion of the effort is to be subcontracted). Any use of subcontractors shall have a written agreement. Verbal agreements are not acceptable. If the effort will not be subcontracted and the prime contractor name and/or address do not match the name and /or address on the provided health inspection report, a technical description shall be provided in sufficient detail explaining why there is a difference in names and locations. Addendum to Evaluation - Commercial Product and Commercial Services

52.212-2 Evaluation-Commercial Products and Commercial Services (Nov 2021)

Evaluation-Commercial Products and Commercial Services (Nov2021)

(a) The Government will award a fixed price contract resulting from this solicitation. Quotes will be evaluated for technical acceptability utilizing the following factors and the award will go to the lowest priced technically acceptable quo

Factor 1 - Health Inspection Report: One legible copy of the state or local government health inspection report for the proposed food preparation facility, if and as required by state and local regulations. The health inspection shall be conducted less than 12 months from the quote submission due date. The facility must have received a score of "A" or "B"; 85 or above" or "pass", if pass/fail inspection system is used. The score must be clearly annotated on the report. Failure to provide an acceptable health inspection report will render the quote submission unacceptable.

Factor 2 - The Government will evaluate past performance on an acceptable/unacceptable basis to determine past performance acceptability. The past performance evaluation is an assessment of the offeror's probability of successfully performing the required effort; therefore, past performance evaluation is not a determination of contractor responsibility.

The past performance assessment is based on the offeror's record of recent and relevant past performance with respect to the services specified in the solicitation requirements. There are two aspects to the past performance evaluation: (1) the recency and relevancy of the past performance, and (2) the quality of that past performance. Past performance is recent if it is within the previous 36 months. Past performance is relevant if it involves contracts valued at a minimum of $25,000.00.

Adjectival Rating / Description

Acceptable: Based on the offeror's performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown (see note above).

Unacceptable: Based on the offeror's performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.

Note: In the case of a company without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the company may not be evaluated favorably or unfavorably on past performance but shall be determined to have unknown (or "neutral") past performance. In the context of acceptability/unacceptability, a neutral rating shall be considered acceptable.

(b) Options. The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quotation is unacceptable if the option prices are significantly unbalanced. Evaluation of option shall not obligate the Government to exercise the option(s).
