About this file
This is a Performance Work Statement (PWS) for an Indefinite-Delivery/Indefinite-Quantity (IDIQ) contract providing 24/7/365 telemedicine clinical support services to five rural hospitals in South Dakota and North Dakota: Pine Ridge Hospital, Rosebud Hospital, Eagle Butte Hospital, Quentin Burdick Hospital (Belcourt, ND), and Fort Yates Hospital (Fort Yates, ND). The contract encompasses two primary service lines: Tele-Emergency (Tele-ED) high-acuity emergency support and Telemedicine Sexual Assault Medical Forensic Evaluation (TeleSAMFE/TeleSANE) support, with services ordered via task orders issued by the Contracting Officer. Tele-ED services provide real-time consultative guidance for emergent events including cardiac/respiratory arrest, trauma resuscitation, airway emergencies, sepsis stabilization, pediatric emergencies, stroke and neurologic events, toxicology/overdose cases, obstetric emergencies, and mass casualty situations. TeleSAMFE/TeleSANE services deliver on-demand support for sexual assault evaluations, including trauma-informed patient engagement, medical and assault history guidance, injury identification, forensic evidence collection, prophylaxis guidance, and safety planning.
The contract specifies a base period of 12 months with four optional 12-month renewal periods. Performance standards mandate Tele-ED initial response within 10 minutes with 95% monthly acceptance quality level (AQL), TeleSAMFE response within 15 minutes at 95% AQL, escalated response within 15–25 minutes at 99% AQL, zero unplanned coverage lapses, documentation completion within 24 hours at 95% AQL, and 99.5% monthly technology uptime. Contractor personnel must include board-certified emergency physicians or equivalent clinicians for Tele-ED with current ACLS/PALS certification, and SANE-certified clinicians with minimum two years SAMFE/SANE experience for TeleSANE services. All clinical staff must be credentialed/privileged by each spoke site, with proxy credentialing permitted where allowed. The Contractor must maintain a Quality Control Plan, conduct monthly and quarterly reporting on consults, response times, technology issues, and de-identified case reviews, and participate in periodic quality improvement meetings. Contractor operations must be remote within the United States using a secure, FedRAMP-approved telehealth platform compatible with hospital systems, with required Business Associate Agreements and privacy/security compliance. Transition-in activities include kickoff meetings, connectivity testing, and go-live readiness within 60 days of award.

C.1 Purpose This Performance Work Statement (PWS) describes requirements under an Indefinite- Delivery/Indefinite-Quantity (IDIQ) contract for 24/7/365 telemedicine clinical support services for five (5) rural hospitals (“Spoke Sites”). The Contractor shall provide (1) Tele-Emergency (Tele- ED) high-acuity emergency support and (2) Telemedicine Sexual Assault Medical Forensic Evaluation (TeleSAMFE/TeleSANE) support, as ordered by task order.
C.2 Background
• Rural hospitals often have limited on-site specialty and critical care coverage and variable access to emergency-trained clinicians.
• Many rural hospitals lack consistent Sexual Assault Nurse Examiner (SANE) coverage, creating delays and inconsistency in forensic evaluations.
• Telemedicine-enabled specialty support improves access to expertise, stabilization, documentation quality, evidence collection consistency, and patient-centered trauma-informed care.
C.3 Scope of Work The Contractor shall provide synchronous audio-video telemedicine support using a hub-and-spoke model for the following service lines, as ordered by task order. The work will support 5 rural hospitals in South Dakota and North Dakota. Approximately ED volumes as follows:

• Surge/mass casualty events upon request
Tele-ED Clinicians provide consultative guidance. On-site clinicians retain responsibility for direct patient care and local clinical decisions.
C.3.2 TeleSAMFE / TeleSANE Support On-demand telemedicine support to enable timely, high-quality Sexual Assault Medical Forensic Evaluations (SAMFE), consistent with applicable standards of care and trauma-informed practices, including:
• Trauma-informed patient engagement, consent guidance, and patient autonomy support
