Solicitation Summary: IHS1527367
This is a Combined Synopsis/Solicitation issued by the Department of Health and Human Services, Indian Health Service, Oklahoma City Area Office for blood products and immunohematology reference laboratory services. The solicitation (IHS1527367) is issued as a Request for Quotation (RFQ) for commercial products under RFO FAR Part 12, with Full and Open Competition and no set-asides. The Indian Health Service seeks to establish a single Blanket Purchase Agreement (BPA) with a firm-fixed-price structure for a five-year period from September 30, 2026 through September 29, 2031, with an aggregate stipulated amount of $325,000 and individual call limitations of $150,000 per purchase. Quotations are due September 14, 2026 at 2:00 PM CST and must be submitted via email to maryann.yocham@ihs.gov. The contractor will deliver all services to Lawton Indian Hospital, 1515 NE Lawrie Tatum Rd, Lawton, Oklahoma 73507, with FOB Destination terms.
The requirement encompasses a comprehensive range of blood products, supplies, and immunohematology testing services including ABO/Rh typing, antibody identification and titers, compatibility/crossmatch testing, extended red cell antigen typing and molecular typing, and prepared blood components such as leukoreduced RBCs, irradiated RBCs, CMV-negative units, apheresis platelets, and frozen plasma. The contractor must meet specific response time requirements: one-hour STAT response plus delivery, four-hour non-routine and eight-hour routine compatibility testing, and six-hour immunohematology reporting. Additional requirements include maintaining standing-order inventory with scheduled change-outs and credit-back for returned units, providing 24/7 telephone coverage, accepting verbal emergency orders, designating an empowered point of contact, and offering consultation services. Evaluation will prioritize the offeror's ability to provide the complete range of blood products with minimal gaps or exceptions, meet turnaround times, maintain sufficient inventory, and provide comprehensive support services, with non-price factors considered more important than price. Past performance will be evaluated based on work performed within three years of the RFQ date in similar scope and complexity.

1. INTRODUCTION
This is a combined synopsis/solicitation for commercial products prepared in accordance with
RFO FAR part 12, using the combined synopsis/solicitation format at RFO FAR 12.202(b), as revised under the Revolutionary FAR Overhaul and the implementing HHS/IHS class deviation, and the simplified procedures for commercial products and commercial services at
RFO FAR 12.201-1. This announcement constitutes the only solicitation; quotations are requested and a separate written solicitation will not be issued. Solicitation number IHS1527367 is issued as a Request for Quotation (RFQ) for Blood Products &
Immunohematology Reference Laboratory. The associated NAICS code is 621991 — Blood and
Organ Banks (size standard $40.0 million).
RFO note: RFO FAR 52.212-3 is removed. Representations and certifications are obtained through the offeror’s active SAM.gov registration; required clauses are drawn from the RFO FAR
12.205 clause tables.
Anticipated contract type. The Government anticipates establishing a single blanket purchase agreement (BPA) (RFO FAR 12.201-1(e)(3)(i)(B)) to fill anticipated repetitive needs.
All calls placed under the resulting agreement will be issued on a firm-fixed-price basis (RFO
Solicitation IHS1527367 Page 2 of 7
FAR 12.104(a)). A BPA is not a contract and obligates no funds; the Government is obligated only to the extent that authorized calls are actually placed (RFO FAR
12.201-1(e)(3)). Quantities stated in this solicitation are estimates for evaluation purposes only and do not represent a guarantee of any volume of business.
2. SET-ASIDE DETERMINATION
The BPA will be established using Full and Open Competition under RFO FAR part 6 — full and open competition with subsequent calls placed off of the single source competed BPA.

Solicitation IHS1527367 Page 3 of 7
Dollar limitation per call. The dollar limitation for each individual purchase under this BPA is $150,000.
Authorized callers. Calls may be placed only by Any authorized Contracting Officer for the Oklahoma City Area Office (OCA
O). A list of authorized individuals will be incorporated into the resulting agreement.
Delivery and shipment documentation. Each delivery or shipment must reference the BPA number, the call number, the line item(s), quantities, and the delivery location.
Invoicing. A summary invoice shall be submitted at least monthly, or upon expiration of this BPA, whichever occurs first, for all deliveries made during the billing period. The summary invoice shall identify the delivery tickets covered, state their total dollar value, and be supported by receipt copies of the delivery tickets, submitted in accordance with RFO FAR 52.212-4.
Clauses. The provisions and clauses listed in Section 7 apply to purchases under this BPA (RFO FAR 12.201-1(e)(3)(iv)(G)).

The requirements at RFO FAR 12.102 and 12.103 apply to each call placed under this agreement.
Calls will be placed directly with the supplier by an authorized individual; the Government does not anticipate competing individual calls under this agreement. This agreement is not exclusive.
The Government may satisfy any requirement within its scope by other means, and nothing in this agreement guarantees any volume of business.
5. EVALUATION METHODS
The Government will evaluate quotations and make award using the method below. This language constitutes the tailored fill-in for RFO FAR 52.212-2, Evaluation—Commercial
Products and Commercial Services (RFO FAR 12.205(a)(2)) and includes all evaluation factors and criteria for award.
Evaluation. The Government will award a purchase order to the responsible quoter whose conforming quotation is most advantageous to the Government, based on a comparative evaluation of quotations against the following factor(s): Quotes will be compared on the offeror's demonstrated ability to:; Provide the complete range of blood products, supplies, and immunohematology testing listed in Section 2, 4, 12, & 13 of the SOW, with the fewest gaps or exceptions.; Meet or exceed the required response and turnaround times in Statement of Work
Sections 4, 12, and 13 (1-hour STAT response plus delivery; 4-hour non-routine / 8-hour routine compatibility testing; 6-hour immunohematology reporting; and the specific red cell and antibody-ID windows).; Maintain sufficient standing-order inventory with scheduled change-
Solicitation IHS1527367 Page 4 of 7 outs and credit-back for returned units.; Provide 24/7 telephone coverage, acceptance of verbal emergency orders, an empowered POC, and consultation services., and price. The Government will compare quotations directly against one another under each factor to determine which provides the greatest benefit to the Government; quotations will not be assigned numerical scores or adjectival ratings, and no competitive range will be established. The non-price factors, when combined, are more important than price. The Government may make award without communications, or may communicate with one or more quoters and request revised quotations. Award will represent the best value to the Government, price and other factors considered. (RFO FAR 12.203(c)(2))
Past Performance. The Government will evaluate the quoter's recent and relevant past performance as an indicator of its ability to perform this requirement. “Recent” means performed within 3 years of the date of this RFQ; “relevant” means similar in size, scope, and complexity. The Government may consider information from any source, including CPARS, references, and the quoter's experience as a subcontractor. A quoter without a record of relevant past performance will not be evaluated favorably or unfavorably on this factor.
Price. The Government will determine price to be fair and reasonable, based whenever possible on adequate price competition. Because this is a Blanket Purchase Agreement establishing unit prices for firm-fixed-price calls, and no guaranteed quantities are stated, the Government will evaluate price on the basis of the unit prices offered across all line items in Attachment 2, Price
Schedule, rather than by computing an aggregate extended total. Prices will be evaluated inclusive of transportation charges to destination (F.O.B. Destination). In comparing quotations, the Government may consider the completeness of a quoter's pricing across the required line items and the reasonableness of individual unit prices, and may take into account the relative importance of higher-volume or mission-critical items. Prompt payment discounts will not be considered in the price evaluation.
Quotation is not an offer. A quotation is not an offer and cannot be accepted by the
Government to form a binding contract. Any resulting purchase order is the Government's offer to the quoter; a binding contract is formed only when the quoter accepts the purchase order, either by written acceptance or by commencing performance. (RFO FAR 12.201-1(b))
Late quotations. The Government will consider all quotations received by the date and time specified above and may, consistent with good business judgment, consider a quotation received after the due date and time. (RFO FAR 12.203(c)(1))
Brief explanation to unsuccessful quoters. Upon request, the Government will provide an unsuccessful quoter a brief explanation of the award decision that explains why that quoter was not selected. If an award notice was not required to be posted to the Governmentwide Point of
Solicitation IHS1527367 Page 5 of 7
Entry, the Government will also provide the information that would have appeared in an award notice. (RFO FAR 12.301(b); see RFO FAR 5.301(c))
6. INSTRUCTIONS TO OFFERORS
Quotations are due no later than September 14, 2026 at 2:00 PM CST. Email is the only acceptable method of submission. Submit quotations by email to maryann.yocham@ihs.gov. Quotations submitted by any other method will not be considered.
