About this file

This is a Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) for outcome-based reagent supply services for an Abbott I-STAT analyzer at Pine Ridge Indian Hospital Laboratory in Pine Ridge, South Dakota.

The contract objective is to ensure continuous, uninterrupted availability of fully compliant reagent supplies for 24/7 clinical diagnostic services with zero disruption due to supply failure, full cold-chain compliance, delivery performance aligned with laboratory demand, and 100% product usability upon receipt. The contractor must provide end-to-end supply availability including sourcing, quality assurance, cold-chain management, and delivery of all required reagents, calibrators, and controls. The estimated annual usage includes approximately 450 troponin tests, 1,400 ABG tests, and 400 Chem8 tests. Specific Abbott catalog items required include CG4+ Reagent Box/25 (3P85-51), Chem8 Reagent Box/25 (09P31-26), Tri Control Levels 1-3 (05P71-01, 05P72-01, 05P73-01), Troponin Reagent Box/25 (03P90-25), and Troponin Control Levels 1-3 (06P17-09, 06P17-10, 06P17-11). Deliveries must be made to Pine Ridge Indian Hospital Laboratory, 607 Indian Health Road, Pine Ridge, SD 57770.

Performance metrics include zero tolerance for stockouts impacting testing, on-time delivery of ≥95%, product acceptance rate of ≥98%, 100% shelf-life compliance with longest available dating, zero tolerance for cold-chain temperature excursions, and 100% single-lot consistency per shipment. The contract period is 12 months from date of award. Acceptance criteria require products within acceptable expiration range, verified temperature compliance, correct items and quantities, single lot numbers, and no visible damage. The Government will conduct outcome-based surveillance through inspection at receipt, inventory tracking, deficiency reporting, and trend analysis. Non-performance remedies include mandatory replacement at contractor expense, shipment rejection, negative CPARS evaluations, non-exercise of options, and potential termination for cause. Critical failures requiring immediate action include any stockout impacting patient care, cold-chain breaches, or repeated delivery failures. Invoicing is performance-based and contingent upon meeting all performance outcomes, with payment reflecting only accepted deliveries. Evaluation will follow a Lowest-Priced Technically Acceptable (LPTA) methodology based on technical capability (ability to obtain genuine Abbott supplies, supply chain capability, cold-chain management, product quality, and delivery performance), past performance (minimum two recent relevant references within three years), and price (completeness, fairness, mathematical accuracy, and compliance with requirements).
