About this file

This is a Questions and Answers document for federal solicitation IHS1519284_IHS1519285_CCHCF, addressing clarifications regarding the submission requirements and operational details for Hospitalist and Nocturnist provider services at a federal healthcare facility (CCHCF).

Submission and Administrative Requirements: Offerors must submit five separate volumes: Volume 1 (Administrative Documents), Volume 2 (Past Performance, 8 pages maximum), Volume 3 (Technical Capability of Vendor, 6 pages maximum), Volume 4 (Qualification Requirements of Key Personnel), and Volume 5 (Price, 2 pages maximum). All documents including CPARS reports and Past Performance Questionnaires count toward the page limits. The government reserves the right to award to one or multiple vendors and permits partial quotes for either the Hospitalist or Nocturnist position depending on whether proposed key personnel meet the specific shift requirements (day shift or night shift). Pricing should reflect grand totals for all option periods using the 520-hour quarterly estimate, though this represents an estimate rather than a fixed requirement. Current incumbent contractors are identified under contract numbers 75H71024P00675, 75H71024P00676, and 75H71024P00677.

Operational Requirements: The positions require a mandatory 7-on/7-off schedule with no alternative rotations accepted. The facility currently operates with one full-time permanent Hospitalist (with 0.5 FTE contractor support requested) and one full-time permanent Nocturnist (with one half-time contractor currently confirmed through September/October). The Hospitalist manages approximately 8 patients per shift with 4 average daily admissions; the Nocturnist cross-covers approximately 16 patients with 3 average admissions per night shift (range 0-6). The facility has 18 floor beds and 4 ICU beds (open, relatively low acuity), and the Nocturnist covers the ICU with limited ventilator management. No outpatient duties are required. Providers may be called to assess patients for admission from Urgent Care and Emergency Room but do not provide scheduled coverage there. Government quarters are available at prevailing rental rates plus utilities with a current waiting list; the facility has limited specialty services onsite and typically transfers patients requiring subspecialist consultation to Flagstaff Medical Center or Phoenix-area hospitals.

Questions Regarding Solicitation Notice ID: IHS1519284_IHS1519285_CCHCF

1. The solicitation cover identifies the solicitation number as IHS1519284_IHS1519285, but the attached Past Performance Reference Questionnaire references IHS1529284_IHS1529285. Please confirm the correct solicitation number.

IHS1519284_IHS1519285 is correct and PPQ should be IHS1519284_IHS1519285.

2. The solicitation uses "Volume 2" as the label for both the Past Performance section and the Technical Capability of Vendor section, and "Volume 3" for both Qualification Requirements of Key Personnel and Price. Please confirm whether these should be relabeled as five separate volumes (e.g., Volume 1 Admin, Volume 2 Past Performance, Volume 3 Technical, Volume 4 Key Personnel, Volume 5 Price). Yes, please submit as follows: Volume 1 Administrative Documents, Volume 2 Past Performance, Volume 3 Technical Capability of Vendor, Volume 4 Qualification Requirements of Key Personnel, Volume 5 Price.

3. Section 52.212-1 specifies a page limit for Past Performance (8 pages), Technical Capability (6 pages), and Price (2 pages), but no limit is stated for the Qualification Requirements of Key Personnel volume. Please confirm whether a page limit applies and, if so, what it is. You may submit qualifications, however, must relate to qualification requirements.

4. Within the 8-page Past Performance volume, are the CPAR reports and the completed Past Performance Questionnaires both counted toward the page limit, or are the questionnaires considered attachments outside the 8-page cap? You can submit. Please consider the page limit and all documents relate to Past Performance, CPARS and PPQ.

5. Solicitation No. IHS1519284 and IHS1519285 appear to reference two requirements (one Hospitalist, one Nocturnist). Please confirm a. Are these two separate awards, or a single combined award covering both positions? Government reserves the right to award to one vendor or multiple vendor b. May an offeror submit a quote for only one of the two positions, or must both be quoted? Depends on key personnel depending on the shift hours day shift or night shift and meets the requirements. c. Should the Schedule of Items hourly rates and 520-hour quantities be priced per position or as a combined total? Yes, Complete each option period with the grand total for all option periods.

6. May the same Key Personnel be proposed against both the Hospitalist and Nocturnist requirements (with different shift assignments), or must distinct candidates be submitted for each? Depends on key personnel depending on the shift hours, day shift or night shift and meets requirements.

7. The Schedule of Items lists 520 hours per 3-month option period. A single provider working a 7-on/7-off, 12-hour-shift rotation yields approximately 546 hours per quarter (84 hrs × 6.5 wks). Please confirm whether 520 hours represents one provider in a 7/7 rotation, partial coverage, or another assumption the Government has used to build the quantity.

520 hours is an estimate.

8. Is there a current incumbent contractor providing Hospitalist and/or Nocturnist services at CCHCF? If so, please provide the contract number(s), period of performance, and total contract value(s). 75H71024P00675, 75H71024P00676, 75H71024P00677.

9. How many Hospitalist and Nocturnist providers (both Federal/Tribal employees and contractors) are currently staffed at CCHCF, and how many FTEs is the Government targeting for full coverage? Overview of our inpatient services: 2 providers on during days (1 hospitalist working 8am-8pm and 1 Internal Medicine or Family Medicine outpatient-based provider working 8am-5pm). 1 nocturnist at night from 8pm to 8am.

Hospitalist: we have one full-time hospitalist (permanent employee). We would be 'fully staffed' with a second full-time hospitalist alternating with the permanent employee but due to financial constraints we are only requesting a 0.5 FTE hospitalist contractor. Our Internal Medicine/Family Medicine providers cover the gaps. Nocturnist: We have one full-time nocturnist (permanent employee) and one half-time contractor currently (only confirmed through September/October). We would be fully staffed with a second full-time nocturnist, alternating with the permanent employee.

10. How far in advance are provider schedules typically built and confirmed (e.g., 30, 60, 90 days)? The inpatient schedule is made in 4-month blocks about 3 months before the start of that block (for example we are working on the September-December block now). We do often adjust and have flexibility to make changes ~30 days out.

11. What is the average daily inpatient census per Hospitalist (ACU and SCU combined), and the average number of new admissions per Hospitalist during a 12-hour day shift?

Average 4 admissions during day shift (although some are admitted by the Internal Medicine/Family Medicine provider). Average 3 patients per night shift.

12. What is the average cross-cover census, the average number of new admissions, and the average number of pages/calls per Nocturnist during a 12-hour night shift? For a nocturnist 12-hour shift, average cross cover census 16 (typical range 10-20); average number of admissions 3 (typical range 0-6); average pages/calls including admissions/potential admissions 10.

13. PWS Section 6.1.1 states the Key Personnel "shall provide coverage in the Urgent Care and Emergency Room Departments." What is the average frequency and duration of such coverage during a typical week, and is this overflow only or scheduled coverage? Majority of patients are admitted from Urgent Care or the ER. Patients are occasionally directly admitted from clinic. Providers will not necessarily provide coverage in Urgent Care and ER but may be called to assess a patient for admittance.

14. The PWS cover page requires a minimum commitment of "7 days on / 7 days off for at least 4 weeks at a time." Will the Government accept alternative rotations such as 14-on / 14-off if the offeror can guarantee 100% coverage of the contracted hours? No because the shifts need to alternate with our full-time providers.

15. PWS Section 5.6.5 states that government quarters are available at "prevailing rental rates plus utilities." Please provide the current monthly rental rate and average monthly utility cost for available quarters, and indicate the typical unit type (e.g., studio, 1-BR, shared housing). Will have to ask Housing Department.

16. Is on-campus housing reliably available at the start of a placement, or is there typically a wait list? If a wait list exists, what is the current expected wait time? Currently, there is a waiting list and priority list for housing.

17. How many beds? Currently 18 floor, 4 ICU.

18. Does the Nocturnist have to cover the ICU, or do they have dedicated intensivists? Yes, they cover the ICU. It is relatively low acuity ICU and those needing intensivist consultation are transferred to outside facility.

19. What is the average amount of admissions overnight? Average 3 admissions per night shift. Typical range is 0-6.

20. Is there a call room available for the nocturnists to use if there are no admissions?

Unfortunately, no but there is a small office.

21. Is the seven-on-seven off schedule mandatory? Strictly 7 on, 7 off.

22. Does the facility currently have an incumbent contractor for this solicitation, or is this a new position? 75H71024P00675, 75H71024P00676, 75H71024P00677.

23. What specialties and support staff are available onsite, including intensivists, APP coverage, or other backup support? We have limited specialty services- currently only Podiatry, General Surgery, OB/GYN, and ID. We do not have intensivist or APP coverage.

24. Which specialties require patient transfer, and which hospitals typically receive those transfers? We typically transfer patients needing any subspecialist consultation including Pulmonology, Cardiology, Nephrology, Neurology, to name a few. We typically transfer patients to Flagstaff Medical center or various hospitals in Phoenix including Banner.

Occasionally we transfer patients to Colorado or New Mexico hospitals.

25. What procedures will the provider be expected to perform? None required. Prefer LP, paracentesis, thoracentesis, central lines. There is backup support from Surgery, ED and anesthesia for these procedures.

26. Is the ICU open or closed? Open, relatively low acuity ICU. Typically transfer patients needing intensivist consultation. Typical patients admitted to our ICU include DKA, alcohol withdrawal. Limited ventilator management; most intubated patients are transferred out.

27. What is the average patient census for both day and night coverage? The daytime hospitalist typically has a census of around 8 and then cross-covers the ~8 patients of the other day-time person from ~5pm to 8pm. The nocturnist cross covers all ~16 patients.

28. Approximately how many admissions occur per shift? Average 4 admissions during day shift (although some are admitted by the second daytime person which is an Internal Medicine/Family Medicine provider). Average 3 admissions per night shift. Typical range is 0-6.

29. Are the shifts strictly 7 on, 7 off, or is there scheduling flexibility such as 2 weeks on, 2 weeks off? Strictly 7 on, 7 off.

30. What is the outpatient scope of the position? No outpatient duties.
