About this file

This is a pricing schedule template for nonpersonal services at Northern Navajo Medical Center (NNMC). The schedule requires pricing for two line items of nonpersonal services, each estimated at 160 hours per month at an all-inclusive regular/overtime hourly rate.

The rate must include all labor, fringe benefits, transportation, per diem, supervision, housing, and other expenses including all applicable federal, state, and local taxes. Holiday and overtime pay are included in the all-inclusive rate. The template notes that the specified hours are estimates only and variations from these estimates will not constitute grounds for price adjustments. The document requires contractor representative signature, date, and DUNS number for submission.
