Statement of Work Consultation Services

1. Scope:

a. Background and Purpose:

Young People under 18 years of age comply half of the population of Shiprock Service Unit (SSU) with over 10,000 young people in it between the ages of 10 and 18.

Comprehensive School Health Education is a key priority area for SSU Health Promotion effort as well as Navajo Nation Dept. of Health, Health Education Program. The key causes of morbidity and mortality for young people in the Shiprock agency are all behaviorally related, preventable. Injuries both unintentional (i.e. motor vehicle crashes) and intentional (homicide, suicide, assault) are the #1 cause of mortality. Client illnesses developing later in life such as diabetes and hypertension have their roots in behavioral begun in childhood.

The SSU Health Promotion Disease Prevention (HPDP) Program is strongly committed to

Comprehensive School Health Education Program including specific health education curricula, focus program targeting key areas morbidity and mortality, and school base health services.

2. Requirements

a. Tasks

1. Contractor shall plan, develop and implement Teen Parent Educator to provide support for teen parents ensuring that they pursue pre-natal care with Indian Health Service (IHS), as well as providing early health and wellness intervention for infants of teen parents.

2. Contractor shall provide direct consultation in the adaptation of health services for Impact Concussion equipment training for Central Consolidated School District (CCSD).

3. Contractor shall provide wellness consultation and access to exercise equipment and swimming pool at Natatorium for all Shiprock IHS patients.

4. Contractor shall provide support in community based HPDP promotional events related to wellness initiative and other HPDP related projects.

5. Contractor shall provide support in Mental health wellness initiatives.

6. Contractor shall develop school and community early response to heart disease programs that will include Automated External Defibrillators (AED) and staff and community training at school sites. Maintain emergency response equipment for first aid kits, AED equipment, and EPI pens for schools and community.

7. Contractor shall provide Crisis Prevention Training for staff to de-escalate inappropriate student behaviors.

8. Contractor shall provide Health training and supplies for students, parents, community and staff.

b. Reporting Requirements

Contractor shall provide monthly activity summaries to Director of Community Health, Shiprock Service Unit including review of activities in each of the eight (8) task areas, specific reference to progress made, problems encountered, and evaluation undertaken.

c. Deliverables

Contractor shall provide a close-out summary report when contract ends.

d. Schedule

Upon award of contract vendor will initiate project and carry out over eleven (11) months after the date of contract award.

e. Period of performance

Project is estimated to take eleven (11) months.

f. Government property

No government property to be used.

3. Quality Assurance Provisions

These will include inspection of preliminary and final materials delivered to Director of Community Health Services, Shiprock Service Unit and review of interim reports by Community Health Services staff. Progress monthly reports between contractor and Director of Community Health Service during the performance period of the contract.

4. References

CCSD is the largest public school district within the Shiprock Service Unit and provides education to a majority of the children within the Service Unit. Many of the previous efforts at health promotion and comprehensive school health education within Service Unit have focused only on the BIA schools, thereby leaving out most of the students within our Service Unit.

Close consultation incorporation with the largest school district within the Service unit is required if we are to reach a majority of our students.

Summary

Contractor shall provide consultation services in the development of programs supporting comprehensive school health education within Shiprock Service Unit as part of the ongoing SSU health promotion plan.
