Health Promotion Disease Prevention (HPDP) Phoenix Area IHS Wellness Program Office of Health Programs, PAIHS

STATEMENT OF WORK

1.0 General

This is a non-personal service contract to provide wellness activities to the staff of the Phoenix Area Indian Health Service (PAIHS). The Contractor shall furnish all the necessary services, qualified personnel, transportation and supervision of contractor personnel, materials, and equipment not otherwise provided by the Government under the terms of this contract to perform the services.

2.0 Background

The Phoenix Area Indian Health Service (PAIHS) is located on floors 5, 6 and 7 of the Renaissance Two Building, 40 N. Central, Phoenix AZ 85004. The PAIHS Health Promotion Disease Prevention (HPDP) Program has a Wellness Program that promotes employee wellness. Employees who wish to participate in the Wellness Program must register. Employees are primarily of Native American decent.

The program utilizes three rooms for all employee wellness activities:

· 7th floor - Fitness Room, equipped with the following exercise equipment:

· 2 treadmills

· 2 ellipticals

· 1 exercise bike

· 1 universal gym

· 10 yoga mats

· 2 Bosu balls

· 1 stretch mat

· 3 stability balls

· 1 weight scale

· 1 stretch trainer

· 6th floor large conference room which can accommodate up to 80 people.

· 6th floor OHP conference room which can accommodate up to 30 people.

3.0 Description of Services

The Contractor shall furnish all equipment and all other items needed to conduct activities listed in this Statement of Work, on-site at the PAIHS or off-site at location convenient for employees. The equipment listed above may be available for contractor performance; however it is not adequate to conduct all activities required.

4.0 Scope

The Contractor shall perform the following services during a twelve (12) month period (from date of award through expiration):

Task 1 – PAIHS office communication with contractor

The contractor shall provide wellness program information/activity at each All-Staff meeting that occurs each month on-site at PAIHS office. These communication seminars may be between 15 to 30 minutes in duration. Contractor shall also provide wellness program information at departmental meetings to increase employee participation. These communication sessions may be between 15 to 30 minutes in duration. Contractor will work with department managers to schedule date and time. Contractor will provide a total of 24 communication seminars throughout the contract period.

Task 2 – Health Risk Assessment (HRA) and Physical Fitness Test (PFT)

· Utilize HRA to provide registered employee with an evaluation of their health and quality of life. The contractor may use a validated HRA used in worksite wellness programs. HRA will be approved by HPDP coordinator before administrator. HRA administration must be electronic or online method most suitable to capture data. HRA must be from the same HRA provider through the contract period for valid data analysis and collection.

· The HRA must obtain the following minimum information but is not limited to:

· Demographic characteristics – age, gender

· Lifestyle – exercise, smoking, alcohol intake, diet

· Personal and family medical history

· Physiological data – weight, height, blood pressure, cholesterol

· Attitudes and willingness to change behavior in order to improve health

· Conduct a Physical Fitness Test (PFT) to measure the registered employee’s knowledge, skill, aptitude and physical fitness level. At a minimum, the contractor shall capture the following but is not limited to:

· Cardiovascular Evaluation: To be determined by contractor and HPDP program

· Muscular endurance/strength test: To be determined by contractor and HPDP program

· Flexibility Measurement: To be determined by contractor and HPDP program

· Body fat composition: skin fold test and girth measurements

· Biomechanics evaluation: To be determined by contractor and HPDP program

· Conduct a total of two Health Risk Assessments (HRAs) and two Physical Fitness Tests (PFTs) per registered employee. HRA 1 and PFT 1 will be completed when the employee begins the Wellness Program. HRA 2 and PFT 2 will be completed at the end of the contract period.

· Within 30 days of request from the HPDP Program, the contractor shall meet with the registered employee, on-site at PAIHS or off-site at a convenient location to complete either HRA 1 and PFT 1 or HRA 2 and PFT 2.

· After each HRA, provide employee with individualized feedback and recommend at least one intervention to promote health and sustain function and/or prevent disease. Contractor will provide health coaching to each registered employee (if desired) to stay on track with recommendations. Contractor will provide follow up and chart progress.

· After each PFT, provide employee with individualized feedback, prescribe physical activity regimen, set goals and evaluate progress. Contractor will provide health coaching to each registered employee (if desired) to stay on track with recommendations. Contractor will provide follow-up and chart progress.

· Within 30 days after the end of the contract period, provide an aggregate report comparing HRA 1/PFT 1 to HRA 2/PFT 2 on each registered employee and submit to HPDP Coordinator.

· Provide each registered employee with an electronic copy of their HRA (1 and 2) and PFT (1 and 2). The contractor shall keep data from all HRAs and PFTs in a safe and secure location for privacy.

Task 3 – Fitness Center/Classes

· Provide Fitness Center/Classes as described below:

Location: PAIHS 6th floor large conference room, 7th floor fitness room, or other location provided by contractor.

Class Duration:30 minutes to 1 hour.
Participants:Registered PAIHS employees (Limited to 50 employees)
Contractor on-site:To be determined by HPDP program.
Class Equipment:To be furnished by contractor or 7th floor fitness room.
Class Set-up:Location where classes are to be held will be set-up by contactor and if

necessary HPDP program.

Fitness Class Description

· Fitness Classes shall vary in their level of difficulty as described below. Each registered PAIHS employee will have full access to fitness classes/center and all other services offered by contractor. To understand how physical activity improves health, contractor will provide heartrate monitors to measure effort exerted during physical activity. Contractor will track progress and provide recommendations. Contractor will provide 3 updates to HPDP program about fitness center activities/classes in report format. Fitness classes may include the following but is not limited to:

· Provide group exercise focusing on beginner body basics-cardiovascular, flexibility and muscular strength and endurance. All classes will be modified for all levels and abilities. Class shall include the following but are not limited to:

· Introduction to fitness

· Importance of flexibility

· Stretch class

· How to use fitness equipment properly

· Why do we hydrate?

· Provide group exercise focusing on more advanced body basics (cardiovascular, flexibility, muscular strength and endurance). However all classes will be modified for all levels and abilities. Class shall include the following but are not limited to:

· Sit-ups

· Push-ups

· Squats

· Jump rope

· Chair exercises

· Provide group exercise focusing on more advanced body basics (cardiovascular, flexibility, muscular strength and endurance). However all classes will be modified for all levels and abilities. Classes shall include the following but are not limited to:

· Pilates

· Yoga

· Circuit Training

· Zumba

· Line dancing

· Provide group exercise focusing on more advanced body basics (cardiovascular, flexibility, muscular strength and endurance). However classes will be modified for all levels and abilities. Classes shall include the following but are not limited to:

· Fitness boot camp

· Kickboxing

· Weight training

· Kettlebell class

· Provide group exercise focusing on more advanced body basics (cardiovascular, flexibility, muscular strength and endurance). However classes will be modified for all levels and abilities. Class shall include the following but are not limited to:

· Crossfit

· Sculpting classes

· Metcon classes

· Athletic conditioning

· P90X

· Spin/Cycle

Fitness Class Schedule

Base Year – October 2015 to September 2016 (from date of award to expiration) The following days Monday through Friday will be available for fitness classes. Time of classes will also vary based on instructor availability and location.

Task 4 – Weight Loss Seminars and Coaching

· Provide information, through seminars, about healthy eating for weight loss to registered employees. All materials shall be provided by the contractor to conduct seminars. Seminars will be conducted on-site at PAIHS office or off-site at a convenient location for employees. Each seminar will be 60 minutes in duration. A total of 16 seminars will be conducted. Weight loss seminars maybe scheduled throughout the contract period. Seminars may include the following but is not limited to:

· Food shopping

· Healthy snacks

· Quick meals for families

· Protein shakes

· Calories/Food labels

· Portion Size

· Fast food choices

· Cell phone apps about nutrition

· Macronutrients

· Provide coaching to support weight loss success to registered employees. Contractor will provide one on one coaching to generate nutrition plan. Contractor will follow up (progress made, barriers, and successes) with registered participants through the following methods: face to face, telephone or email. Contractor will provide 3 updates to HPDP program about coaching in report format.

· Provide certified expert or experienced consultants for weight loss seminars and coaching. And provide local resources to employees regarding each seminar.

Task 5 – Wellness Challenges

· Provide 3 different wellness challenges for registered employees. All materials shall be provided by the contractor and used during the challenges. Number of participants registered for each Challenge can range from 1 to 50. Challenges should be fun and create enthusiasm for healthier lifestyles. Challenges should be organized for teams or individuals. All Challenge kick-offs will be provided onsite at PAIHS or off-site at a convenient location for employees. Date of challenges will be coordinated with the HPDP program. Challenges will include the following minimum information but is not limited to:

Maintain Don’t Gain (maintaining weight) Feel Fit with Fiber (eating more fruits and vegetables) Lower Your Weight by Eight (cutting calories, increasing physical activity) Quench Your Thirst (Drink more water)

· Challenges may be provided either through paper format, online or other preferred method for participation. Contractor will provide updates and progress to registered employees for the duration of each wellness challenge. Updates will include: number of participants, highlights, progress and end of challenge wrap up (winners, weight loss, etc.). Contractor will provide 3 updates for each wellness challenge to HPDP program in report format.

Task 6 – Stress Management Seminars

· Provide information, through seminars, about stress management, to registered employees. All materials shall be provided by the contractor and used during the seminars.

· Seminar content shall include the following information but is not limited to:

· Massages

· Meditation

· Tai chi

· Team building activities

· Relaxation techniques

· Time management

· Financial management

· Each seminar will be 60 minutes in duration. A total of 6 stress management seminars will be conducted on-site at PAIHS or off-site at a location convenient for employees. Stress management seminars maybe scheduled throughout the contract period.

· Provide certified expert or experienced consultants for each stress management seminar. And provide local resources to employees regarding each seminar.

Task 7 – Physical Health Seminars

· Provide information, through seminars, about aging, to registered employees. All materials shall be provided by the contractor and used during the seminars.

· Seminar content shall include the following information but is not limited to:

· Women’s health issues

· Men’s health issues

· Prescription medications

· Age specific health issues

· Know your body

· Blood pressure management

· Diabetes management

· Each seminar will be 60 minutes in duration. A total of 6 physical health seminars will be conducted on-site at PAIHS or off-site at a location convenient for employees. Physical Health seminars maybe scheduled throughout the contract period.

· Provide certified expert or experienced consultants for each physical health seminar. And provide local resources to employees regarding each seminar.

5.0 Invoicing

Invoices shall be submitted on a monthly basis
