Combined solicitation/synopsis NOTICE TO CONTRACTOR:

This is a COMBINED SOLICITATION/SYNOPSIS for commercial items/service prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to award a Firm Fixed Price (FFP) contract, utilizing procedures under FAR part 13. The proposed contract action is to provide a web-based credentialing

/privileging software package that allows our health facilities to establish or maintain a Credentialing database system to track credentialing and privileging practices at each of the Phoenix Area IHS hospitals and clinics. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number

IHS1234567. This solicitation documents and incorporates provisions and clauses in effect through FAC

2005-88, 16 May 2016. It is the contractor's responsibility to be familiar with the applicable clauses and provisions.

The North American Industry Classification System (NAICS) Code for this acquisition is 518210; Size

Standard is $32.5M.

The Government will award a firm, fixed price contract resulting from this combined synopsis/solicitation to the responsive and responsible offeror whose conforming response is determined to provide the best value to the Government, price and other factors considered. The

Government will only consider firm fixed-price quotations.

Evaluation Factors/Criteria: The major evaluation factors for this solicitation are technical, (which include customer service, experience and qualifications of staff and technical support), past performance and cost/price factors. Although technical factors are of paramount consideration in the award of the contract, cost/price is also important to the overall contract award decision. All evaluation factors other than cost or price, when combined, are significantly more important than cost or price. In any case, the Government reserves the right to make award(s) to that offeror whose proposal provides the best overall value to the Government.

Offerors are advised that award will be made to that offeror whose proposal provides the combination of features that offers the best or greatest overall value to the Government. The Government is more concerned with obtaining performance capability superiority rather than lowest overall cost. However, the Government will not make an award at a significantly higher overall cost to the Government to achieve only slightly superior performance. Overall cost to the Government may become the ultimate factor for award of a contract as proposals become more equal based on other factors.

The evaluation will be based on the demonstrated capabilities of the prospective Contractors in relation to the needs of the project as set forth in the RFQ. The merits of each proposal will be evaluated carefully. Each proposal must document the feasibility of successful implementation of requirements of the RFQ. Offerors must submit information sufficient to evaluate their quotes based on the detailed criteria above.

Offeror to be considered responsible shall have an updated data and information in System for Award

Management (SAM), FAR provision 52.212-3 Offeror Representations and Certifications-Commercial

Items (APR 2012). The provision can be submitted at https://www.sam.gov. Contractor must be registered to the System for Award Management (SAM) prior to award. A nine digit Data Universal

Numbering System (DUNS) number provided by Dun and Bradstreet is required to register.

Electronic submission of Quotes: Quotations shall be submitted electronically by email to tochukwu.aniche@ihs.gov. Email submissions are limited to 3MB. The submitter should confirm receipt of email submissions. Please provide pricing for base year as well as the four (4) one-year option years.

52.217-8 - Option to Extend Services - The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor.

The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 45 days.

52.217-9 Option to Extend the Term of the Contract (MAR 2000):

(a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 66 months.
