About this file

This document is an Indian Economic Enterprise Representation Form for federal contracting opportunities with the Indian Health Service. It requires any offerors responding to solicitation number IHS1483670 for propane services at the Tsaile Health Center to self-certify whether they meet the definition of an "Indian Economic Enterprise" as defined in the HHS Acquisition Regulation. To qualify, the enterprise must be at least 51% owned by a federally recognized tribal entity and meet the ownership criteria both at the time of offer and throughout contract performance if awarded. Successful offerors must also be registered in the System for Award Management. Any false claims of eligibility are punishable by law. This form allows offerors to represent their status in response to the identified solicitation for propane services at the specified location under a potential Indian Economic Enterprise set-aside.
