About this file
The document is an Indian Economic Enterprise (IEE) Representation Form from the Indian Health Service (IHS), used to self-certify a business's eligibility under the Buy Indian Act. The form requires offerors to represent whether they meet the definition of an "Indian Economic Enterprise" with at least 51% Indian ownership, which must be maintained throughout the solicitation, contract award, and contract performance periods.
The form emphasizes legal requirements, including that false or misleading information is punishable under federal law, with potential penalties under 18 U.S.C. 1001 and 31 U.S.C. 3729-3731. Offerors must also be registered in the System of Award Management (SAM). The form must be completed for solicitations under the Buy Indian Act, and individual Contracting Officers may require additional documentation to verify eligibility before awarding set-aside or sole source contracts.
