Summary of IFB_C6669-130304 Attachments for CCC Delta Pre-Enrollment Physicals

This document set comprises the required attachments and certification forms for the State of California's Invitation for Bid (IFB) for pre-enrollment physical examinations at the California Conservation Corps Delta Center. The contract requires the selected contractor to provide comprehensive physical examinations for Corpsmember Trainees, including all necessary labor, materials, tools, and incidentals. Evaluations must assess all Corpsmembers on-site for both CCC Fire Crew and CCC Standard Corpsmember positions, regardless of the participants' stated program interests within the organization. The bid submission process requires completion of nine attachments, including a bid cover page, bid submission checklist, payee data record, contractor certification clause, bidder declaration with identified subcontractors, proof of insurance, and a cost worksheet.

Bidders must provide cost proposals on a per-physical basis, which should include all direct and indirect costs, insurance, and sales taxes. The contract requires proof of current commercial general liability insurance ($1,000,000 minimum), commercial automobile insurance ($1,000,000 minimum), and worker's compensation coverage ($1,000,000 employer's liability per accident). Post-award, certificates of insurance must include specific endorsements designating the State of California as an additional insured and primary, non-contributory party. Bidders with previous international business activities within the preceding three years must complete the Darfur Contracting Act Certification, and those seeking disabled veteran business enterprise (DVBE) incentives must submit corresponding declarations. The bid submission process awards the contract to the lowest responsive bid, with award contingent upon the bidder's organization structure, ownership certification, and compliance with all specified requirements.
