About this file
This document is a Worker Protection and Investment Certification Form for contractors and grantees of the Commonwealth of Pennsylvania. It requires contractors and grantees to certify compliance with various Pennsylvania state labor and workplace safety laws, including the Construction Workplace Misclassification Act, Minimum Wage Act, Prevailing Wage Act, and others listed in the form. The contractor or grantee must certify they are duly authorized to execute the certification and that the entity is compliant with the applicable laws. Providing false certifications can result in contract termination and three-year ineligibility to bid on future contracts.
The related state and local contract opportunity is for the Pennsylvania Liquor Control Board (PLCB) to procure eighteen different versions of branded gift cards for use in over 600 Fine Wine & Good Spirits Stores across the state. The contract will be awarded to a single contractor to provide the gift cards in accordance with the specifications outlined in the Invitation for Bid (IFB). No additional details are provided regarding pricing, set asides, funding sources, or other salient information.
